Person in charge was unable to produce a certified food protection manager certificate. A CFM is required to be on the premises during all hours of operations. Provide a CFM onsite by next routine inspection.

Addl Notes: Observed employee touch hair, utilize personal phone, clean off a table, and then scoop ice for a customer drink without washing hands. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed all handwashing sinks in the bar with food debris and straws inside. Observed employee dump ice/straws from cup into the handwashing sink at time of inspection. Reviewed with person in charge (PIC) informed that hand sinks must be limited to hand washing only to prevent contamination of food contact surfaces/foods with bacteria from hands. Hand washing facilities must be maintained in a condition that promotes handwashing and restricted for that use. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. PIC relocated all items blocking handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed establishment made coconut sauce, habanero salsa, chipotle salsa, and chorizo with no date marks at time of inspection. Per cook, all items were made between 2 to 4 days prior to inspection. All RTE/TCS foods were properly date marked at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Comments:
7-102.11 - Priority Foundation: Common Name-Working Containers Addl Notes: Observed four chemical spray bottles all containing a clear liquid without any identifying labels on the spray bottles. Per person in charge (PIC), the chemical spray bottles contain sanitizer. PIC labeled the bottles at the time of inspection with the common name of the chemicals. Discussed the importance of properly labeling all working containers containing chemicals with the common name of the chemical. Repeat violation (2nd consecutive). Corrective Action: Corrected at time of inspection.
PRIORITY VIOLATION-7-201.11 - Priority: Separation-Storage
Addl Notes: Observed multiple chemical spray bottles containing sanitizer were stored on a table near the bar next to pitchers of tea at time of inspection. Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Discussed proper separation of storage with PIC at time of inspection. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-3-202.16 - Priority: Ice
Addl Notes: Observed bottle of alcohol being stored in ice well that was being utilized for drinks at the bar at time of inspection. Observed bartender scoop ice for a drink out of ice well at time of inspection. PIC removed bottle at the time of inspection. Employee discarded ice, cleaned bin and provided new ice at time of inspection. Any ice used as an exterior coolant cannot be used as food. Discussed the importance of not using ice as an exterior coolant and then using the same ice as food. Corrective Action: Corrected at time of inspection.

- Reviewed requirement with PIC about windows at bar & wall facing Mill Ave to remain shut and not to be left open until air curtains are installed or IPM approval.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
