Addl Notes: By the dishwashing area, observed a large salad spinner stored directly on the handwashing sink. Sinks must be kept clear and available at all times. Salad spinner was moved. Addl Notes: Observed a large bowl of raw beef stored directly on top of several half gallons of milk. Food was also on the middle shelf. On the bottom shelf there was sour cream, pickles, and beer batter. All raw foods must be kept stored below ready to eat foods or on a separate area to prevent any risk of cross contamination. Bowl was moved. No visible contamination was observed. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed a pan of marinara sauce at 154*F on the sides, but holding at 84*F in the middle. Per employees, it was already reheated on the stove and placed in the unit an hour ago to hot hold. All foods holding in hot holding units must be kept at 135*F or above, throughout the entire pan of food. Employee reheated the marinara to 180*F and placed it back in hot holding. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Corrective Action: Corrected at time of inspection.

Addl Notes: 2nd Repeat Priority Violation. In the westmost prep cooler on the cook line, observed a pan cooked Tuna Salad with the datemark of 6/27, 12 days prior. All ready to eat (RTE), Time/temperature controlled for safety (TCS) foods are required to be sold or discarded on or before 7 days. After 7 days, the food must be discarded. Discussed monitoring dates with manager. Food was embargoed. Predefined Comment: All RTE/TCS foods past 7 days of preparation were discarded by PIC at the time of inspection. Predefined Comment: RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed 30+ fruit flies in the back by the three compartment sink and dishwashing machine. Establishment must be maintained free of pests. Please have establishment treated for pests and eliminate any harborage areas prior to re-inspection. Please have pest control documentation available for re-inspection. Predefined Comment: Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection.

3-307.11 - Core: Miscellaneous Sources of Contamination Addl Notes: Observed employee drinks sitting on the metro shelving by the kitchen door. Drinks were above utensils, dishes, and to go containers. All drinks must be stored in a manner that prevents the risk of contamination of food, food contact surfaces, utensils, and to go/single use articles. PIC moved the employee drinks. Corrective Action: Corrected at time of inspection.

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed food debris, trash, residues, and organic matter accumulating on the floors and covings of the walk in cooler. Facility floors, walls, and ceilings, must be cleaned as frequently as needed to prevent any accumulations of soils. Please have floors cleaned and cleaned more frequently. Predefined Comment: Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
