Predefined Comment: To ensure food safety, prevent physical contamination and the transmission of disease; employee eating, drinking, beverage storage, and using tobacco must be done in an approved area. Addl Notes: Observed oysters stored in a plastic container without shellstock tag in the walk-in. Oysters were not in original package, missing shellstock tag, and submerged in water and undrained ice. Predefined Comment: Shellstock tags must be kept with shellstock until sold, dated with final sale date, and maintained in chronological order for 90 days onsite. Predefined Comment: Shellstock without shellstock tags was discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-7-206.13 (A) - Priority: Tracking Powders, Pest Control and Monitoring
Addl Notes: Observed a white powder spread along the floor wall junction throughout the establishment to include the dry storage room, behind prep coolers, behind the cook line, under hand wash sinks, under three compartment sink. The person in charge stated the powder was placed by the pest control company during the most recent pest control service. Discussed a tracking powder pesticide may not be used in a food establishment. Person in charge contacted pest control to remove the powder. Corrective Action: Corrected at time of inspection.
7-209.11 - Core: Storage-Other Personal Care Items Addl Notes: Observed employee personal items such as clothing and purse stored next to clean utensils/food containers in the storage rack next to the 3comp sink. Lockers or other suitable facilities shall be provided for the orderly storage of employees' clothing and other possessions. All personal items were properly stored at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed container of insecticide and powdered cleaning materials stored on shelves with clean utensils. Observed bottle of rubbing alcohol stored next to utensils/food containers and food ingredients on the shelf above the prep table. Predefined Comment: Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

4-903.11(A), (B) and (D) - Core: Equipment, Utensils, Linens and Single-Service and Single-Use Articles-Storing; Utensils, Equipment and Linens Addl Notes: Observed bins filled with ice and water that were stored directly on floor in front of ice machine with the ice scoop stored in one of the bins. Discussed utensils to be stored in a clean, dry location; where they are not exposed to splash, dust, or other contamination; and at least 15 cm (6 inches) above the floor. Clean equipment and utensils shall be stored in a self-draining position that allows air drying; and covered or inverted. Corrective Action: Corrected at time of inspection.

Predefined Comment: Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed accumulation of food debris/splatter and organic matter on the walls, ceiling, and floor of the walk-in cooler. Predefined Comment: Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed section of missing wall panel under the handwash sink by the prep sink. Observed wall panel separation by the handwash sink next to the 3comp sink. Observed areas of unsealed flooring with exposed wood, deep groves, and pitted concrete. Observed broken tile and unsealed flooring at the threshold of walk-in cooler and inside the walk in cooler. Observed missing ceiling tiles and open spaces in ceiling next to ice machine and dry storage rack. Observed outer opening from gap under back door threshold. Predefined Comment: All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. "A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
