Addl Notes: At the expo station, observed employee grab tortillas with their bare hand and place them on the plate, ready for service. Predefined Comment: RTE food that was contacted by bare hands was discarded by PIC at time of inspection. Predefined Comment: Discussed the importance of preventing bare hand contact with ready-to-eat (RTE) foods with the person-in-charge (PIC). Bare hand contact is not approved with ready-to-eat (RTE) food items. Corrective Action: Corrected at time of inspection.

Addl Notes: At the outside smoker handwashing sink and the handwashing sink in the dishwashing area, no soap/hand cleanser was available at time of use. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Predefined Comment: PIC provided soap for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: In the prep unit across from the grills on the food prep line, observed tomatillo salsa at 53*F, guacamole with tomatoes at 49*F, cheese sauce at 50*F, cooked potatoes at 52*F, cooked garlic at 52*F, sliced tomatoes at 50*F, and roast beef at 49*F. All foods that had been in the unit less than 4 hours were moved to the walk in cooler. Foods that had been in the unit over 4 hours were embargoed. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

3-501.13 - Core: Thawing Addl Notes: In the drawer cooler under the char grill, observed salmon fillets thawing while still in the ROP. ROP stated on the package to remove from package prior to thawing and "keep frozen" Predefined Comment: All ROP fish labeled "Keep Frozen" must be removed completely from reduced oxygen packaging (ROP) prior to thawing under refrigeration or immediately after thawing under cold running water. All ROP fish was removed from ROP packaging at time of inspection. Corrective Action: Corrected at time of inspection.

6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed rodent droppings at the following locations: On shelving under the ansul system in the kitchen, on the hot holding box in the kitchen, on the window sill in the dishwashing area, on the dishwashing machine in the dish area, on the shelving in the dishwashing area, inside and around the 3 compartment sink in the dish area, On the shelving and on food containers in the side storage room, on the floors of the dry storage room, and on the water heater. No pest control report was available at time of inspection. Predefined Comment: Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection.

Addl Notes: Observed a bottle of lemonade stored in a soda fountain ice bin. Per employee the ice is used for consumption. The ice was burnt and the bin was sanitized, then refilled with new ice. Predefined Comment: Ice used as an exterior coolant may not be used as an ingredient in any food/drinks. Corrective Action: Corrected at time of inspection.

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: This is the 5th consecutive violation. Observed grease, food debris, rodent droppings, trash, and other soils accumulating on the floors of the kitchen, under the cooking equipment. Predefined Comment: Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
