Addl Notes: This is the 2nd repeat violation. At time of inspection, no handwashing cleanser was available at the handwashing sink by the dishwashing machine. PIC replaced hand soap dispenser. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-4-501.114 (A-E) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Addl Notes: Observed the bar dishwashing machine in use with 0 ppm of chlorine sanitizer. Upon inspection, the sanitizer, rinse aid, and dishwashing detergent were all empty. Until chemicals are replaced, unit has its lines primed, and is dispensing the proper concentration (50-100 ppm) of sanitizer, establishment will be utilizing the back dishwashing machine. Predefined Comment: Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Corrective Action: Corrected at time of inspection.

Addl Notes: In the hot holding containers on the stove, observed an overfilled container of pork between 117-138*F (majority between 117-120*F). Had PIC split the pork into more shallow portions and place into the hot water bath to reheat. Pork was reheated to 165*F prior to returning to hot holding. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed the both prep coolers in the kitchen out of service. In the coolers observed all foods on ice. Observed coleslaw at 50*F, sliced cheese at 48*F, cooked tomatoes at 47*F, salsa at 46*F, cooked corn salsa at 49*F, bacon at 58*F, guac with tomatoes at 47*F, and broccolini at 49*F. All foods were placed in the unit approximately 3.5 hours prior. Foods were moved to the walk in to rapidly cool. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection. Repeat violation

Addl Notes: On the cook line, both prep coolers were not functional and were being used to store TCS food items on ice. All equipment used for cold holding must be functioning and holding below 41*F or below. Please have these coolers repaired prior to re-inspection. Predefined Comment: Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct prior to re-inspection.

6-202.15 - Core: Outer Openings, Protected Addl Notes: Observed the back door with cracks around the edges and not fully closing. Please have back door replaced. Outer openings must be protected to prevent pest entry. Corrective Action: Correct prior to next routine inspection.
6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: This is the 5th repeat violation. Observed rodent droppings on the floors under the dishwashing machine, on the dishwashing machine, on the dish rack shelving, on the floors of the kitchen, and on cans and containers in dry storage. Observed chewing on a container of seasoning in dry storage. Establishment must remain free of pests. Please have pest issues eliminated prior to re-inspection. Corrective Action: Correct prior to re-inspection.

4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Addl Notes: Observed the handwashing sink in the kitchen falling off of the wall. Basin is at an angle that allows a pool of standing water to accumulate. Water could become an easy water source for pests. All equipment must remain in good repair. Please have sink reattached. Corrective Action: Correct prior to next routine inspection.

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: This is the 6th consecutive violation. Observed large amounts of grease and food debris on the floors under the cooking equipment. Please have area cleaned more frequently. Floors should be cleaned as frequently enough to prevent any soil accumulation. Corrective Action: Correct prior to next routine inspection.
6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed a large hole in the wall behind the ice maker and soda machine in the soda room. In the dry storage room, observed a hole in the wall covered with an aluminum pan lid. Please have holes in the wall repaired and repaired with a smooth, durable, and easily cleanable surface. Corrective Action: Correct prior to next routine inspection.

Predefined Comment: Due to the violations noted on this inspection report, County legal action may result from this inspection.
Re-inspection for the repairs/replacements of the coolers and the rodent activity will be schedule for Tuesday, February 18th. Please send any recent pest control reports to the inspector for review.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
