Description: Food-Contact Surfaces: Cleaned and Sanitized
Comments:
PRIORITY VIOLATION-4-501.114 (A-E) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Addl Notes: The ware wash machine measured at 0ppm for chlorine. Person in charge (PIC) tried to prime and troubleshoot the issue. There was no chlorine observed and PIC called to have machine fixed. PIC moved to the 3-compartment sink at time of inspection. Food-contact surfaces must be properly sanitized after washing and rinsing. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Chlorine needs to be at 50-100ppm. Corrective Action: Correct prior to re-inspection. ***Re-inspection is scheduled for 07/22/2026.

3-501.19 (A1, B2, C2-3) - Priority Foundation: Time as a Public Health Control; Documentation. Addl Notes: Observed multiple containers of TCS food (sprouts, cooked noodles) being stored with ice and with a time stamp. The person in charge (PIC) they were kept on time as a control for safety. The establishment does not have a policy/procedures for time as a control. Discussed time as a control requirement and left PIC a guidance document through email. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Must provide written time as a control procedures prior to re-inspection. Corrective Action: Correct prior to re-inspection. Re-inspection is scheduled for 07/22/2026.

Comments:
7-102.11 - Priority Foundation: Common Name-Working Containers Addl Notes: Observed two condiment containers with cleaners in them. There was no label on the containers. Discussed contamination risk of using food/condiment containers for bleach, and to not have a label to warn people that it is a chemical/cleaner versus oil/condiment. Person in charge (PIC) wrote a label on the containers at time of inspection and will look into new bottles designed for chemical use. Corrective Action: Corrected at time of inspection.
PRIORITY VIOLATION-7-201.11 - Priority: Separation-Storage
Addl Notes: Observed butane, batteries, and cigarettes stored above curry and single service items in the storage room. Discussed proper storage to reduce contamination risk. Person in charge (PIC) properly stored at time of inspection. Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

3-305.11 - Core: Food Storage - Preventing Contamination from the Premises Addl Notes: Observed multiple containers of food stored directly on the ground. Food must be stored at least 6 inches off the ground at all times to prevent contamination from the premises. Corrective Action: Corrected at time of inspection.

Comments:
3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed a small crockpot of water with a rice spoon and other utensils in it. The water was 72*F. Person in charge (PIC) turned the crockpot higher. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed a sprayer nozzle attached to the hose at the mop sink, not under pressure. Person in charge (PIC) removed the sprayer nozzle at time of inspection. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. PIC removed shut-off downstream from atmospheric vacuum breaker at time of inspection. Corrective Action: Corrected at time of inspection.

***Re-inspection scheduled for 7/22/2026 for the ware wash machine and the time as a control policy. I will also be looking at the Food Handler's Certificates for employees as person in charge (PIC) needed to find out where they were and who still needs them.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
