Addl Notes: -- Observed an employee return from a restroom break and begin putting on gloves to continue with food preparation without washing their hands between tasks. Reviewed proper handwashing procedures and requirements with the person‑in‑charge (PIC), emphasizing that hands must be washed whenever contamination may occur and when changing tasks. The employee then properly washed hands at the handwash sink during the inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: 2 of 3 hand wash sinks in kitchen did not have paper towels available for use (hand sink near desk area and hand sink near pizza dough prep area). Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Requested PIC provide paper towels for both handwash sinks at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: -- 2 of 3 hand sinks had various items inside or blocking the hand sink basins. An employee was observed using hand sink water for food prep and mixing pizza dough at hand sink as well. Reviewed hand sink restrictions with the person in charge: • Hand sinks must be maintained free and clear so that it is accessible at all times. • Not used for purposes other than handwashing. • Facilities must be maintained in a condition that promotes handwashing. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. Requested the PIC remove all items blocking hand sink basin at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: -- Time as a public health control timers not being used for 3 pizza racks with about 142 cheese and pepperoni pizzas out at room temperature at the time of inspection. The person in charge stated the pizzas were just prepared less than 30 minutes prior to the inspection. Requested that the PIC properly time stamp all TCS foods being held under time as a control at time of inspection. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Corrective Action: Corrected at time of inspection.

Description: Proper Cooling Methods Used; Adequate Equipment for Temperature Control

Comments:

3-501.11 - Core: Frozen Food Addl Notes: -- In reach-in freezer, multiple bags of TCS foods such as beef, sausage, and ham were softened to touch and had internal temperatures measuring between 30°F-35°F using probing thermometer. The ambient temperature of unit measured at 35.2°F using min/max thermometer. Stored frozen foods shall be maintained frozen unless thawing or slacking. The PIC to notify supervisor to place a work order to repair equipment. Corrective Action: Correct prior to next routine inspection.

Description: Thermometers Provided and Accurate

Comments:

4-302.12 - Priority Foundation: Food Temperature Measuring Devices Addl Notes: -- Unable to verify a working probe thermometer is available on site. The person in charge on duty was unsure where the thermometer would be located. Properly calibrated probe thermometers are required at all times for monitoring food temperatures. Must provide a probe thermometer for establishment prior to re-inspection. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
