Addl Notes: Observed employee handling dirty dishes and then went and handled clean dishes without washing their hands in between. When going from dirty dishes to clean dishes, a full handwash must be completed between. Educated employees on proper hand washing. Instructed employee to rewash dishes. Predefined Comment: Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed no paper towels or other hand drying provisions at the handwashing sinks by the dishwashing area, kitchen, and bar. A hand drying provision must be kept available at all times. Handwashing sinks were all supplied with paper towels at time of inspection. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Corrected at time of inspection.

Addl Notes: In the tall reach in cooler by the cook line, observed raw beef over cream and milk, observed raw whole eggs over cooked chicken. In the rightmost prep cooler, observed raw beef patties over jello shots in syringes. In the walk in cooler, observed a box of raw bacon on top of a box of brioche buns. All foods were moved to proper storage with the raw foods below the ready to eat (RTE) foods. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Comments:
7-209.11 - Core: Storage-Other Personal Care Items Addl Notes: Observed a packaged bottle of toothpaste and tooth brush stored on top of the seasonings. Observed a bottle of tooth paste and a used tooth brush stored on the upper portion of the middle prep cooler. All personal care items must be stored away from food, food contact surfaces, and utensils. Employee moved items to proper storage. Corrective Action: Corrected at time of inspection.
PRIORITY VIOLATION-7-201.11 - Priority: Separation-Storage
Addl Notes: Observed a can of "goo gone" on metro shelving directly above an open container of sugar and closed containers of brown sugar and raw pasta. Chemical was moved away from food. Predefined Comment: Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

3-502.11 - Priority Foundation: Variance Requirement. Addl Notes: Observed 10 cooked wings in each of 15 ROP bags. None of the bags or container had a visible date or time stamp. Because the wings were in ROP bags, a date and time stamp is required to ensure they do not go over the 48 hours. Per employee, they were sure the wings were cooked 2 days prior but were unsure of when they were placed in ROP bags. Because there was no date and time stamp, the bags were opened and date marked for Sunday. Corrective Action: Corrected at time of inspection.

3-501.13 - Core: Thawing Addl Notes: Observed several packages of raw tuna and several packages of raw salmon all in ROP and thawed. All packages stated to remove from packaging when thawing. Instructed establishment to start removing fish from ROP when thawing. Predefined Comment: All ROP fish labeled "Keep Frozen" must be removed completely from reduced oxygen packaging (ROP) prior to thawing under refrigeration or immediately after thawing under cold running water. All ROP fish was removed from ROP packaging at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:
6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: In the back by the mop sink, observed 30+ fruit flies flying around the drain. Observed a bucket of smashed fruit being used to catch the flies however, it appeared to be harboring the flies and allowing breeding conditions. Bucket of smashed fruit was removed. Establishment must remain free of pests. Please have pest issues eliminated prior to re-inspection on Friday May 23rd. Predefined Comment: Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
