Addl Notes: In the walk in cooler, observed raw chicken stored above ground lamb and beef. Observed raw chicken and raw ground beef stored above sealed packages of whole beef roasts. Observed raw hamburger patties stored above cubed steak and lamb chops in the reach in cooler on the cook line. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed food debris on the meat slicer blade guard and the meat grinder blade. Observed a greasy residue on the meat grinder auger. All food-contact surfaces must be cleaned to sight and touch. PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-7-201.11 - Priority: Separation-Storage

Addl Notes: Observed aerosol window cleaner stored on the prep surface next to meat pies that were being restocked and various dry goods. Observed another window cleaner can stored next to dry goods on the prep surface near the gelato cooler. Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

4-602.13 - Core: Nonfood Contact Surfaces Addl Notes: Observed a heavy accumulation of food debris on the solid metal shelving in the walk in cooler. Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

Description: Physical Facilities Installed, Maintained, and Clean

Comments:

6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: Observed two non-NSF or equivalent nut butter grinders in the back kitchen and near the nut, olive and pastry counter. Observed the grinder constructed of a gritty/sandy material that comes loose when scratched. PIC stated the machines were used to grind pistachios but are now purchasing a pre-made product. Advised PIC all equipment utilized must be commercial quality with smooth and easily cleanable surfaces. Advised PIC the grinding elements may cause physical contamination of the nut butter processed with it. Since the equipment is not being used and is not commercial quality, PIC was instructed to removed the machines from the premises prior to the next routine inspection. Corrective Action: Correct prior to next routine inspection.

Due to the violations noted on this inspection report, County legal action may result from this inspection.
Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.
Establishment has expanded equipment in the restaurant area. Establishment has added new plumbing connections for a coffee machine, water filter and ice machine. No backflow observed on the water line. Also observed 2 new coolers utilized in the area. Establishment has also added a gas stove to prepare knafeh in the service counter area and another gas stove in the back near the water heater that are not held under a hood vent. PIC was advised to discontinue use of gas equipment until a proper hood has been installed, or to remove the gas-powered equipment and replace with electric, NSF or equivalent equipment. Advised PIC that not all areas where equipment was added are in direct line of sight of the hand wash sink and additional hand sinks may be required.


Observed 3 comp sink quat dispenser mixing quat at less than 100ppm quat when measured with a chemical test strip. No active warewashing observed at the time of inspection. Advised PIC to manually mix quat or have dispenser repaired. Advised PIC the chemical supply bucket appears to be a different brand than the dispenser manufacturer which may utilize a different concentration supply bucket.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
