Addl Notes: Observed establishment prepare and serve raw ready to eat shrimp ceviche but have no updated parasite destruction letter from seafood provider. Predefined Comment: Parasite destruction records or letter from supplier are required for all non-exempt fish served raw or undercooked at establishment. Must provide parasite destruction letter from supplier prior to re-inspection. Corrective Action: Correct prior to re-inspection.

PRIORITY VIOLATION-7-201.11 - Priority: Separation-Storage
Addl Notes: Observed spray can of WD-40 stored under the prep table next to packaged food, food equipment, and in direct contact with utensils. Predefined Comment: Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Predefined Comment: Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.
Observed establishment conducting outdoor cooking without an approved outdoor food operations approval. Observed an al pastor meat trompo at ambient temperature in a vertical spit broiler on top of a folding table outside the front entrance. Observed a mobile flat top griddle with holding compartments for prepped food items such as toppings and sauces that employees were bring out from inside the establishment. The holding compartments had no cold holding or hot holding capability. The veritcal spit broiler and flat top griddle were connected to mobile propane tanks. Observed a tip jar on top of the flat top griddle and a menu banner on the front of the flat top griddle showing pictures of menu items such as tacos al pastor, tacos de cabeza, tacos de lengua, tacos de maciza, quesadilla de pastor, vampiro pastor, alambre de pastor, with the printed business name "TACO MICH" and instructions to "PAY INSIDE PAGAR ADENTRO." All equipment and food was observed left unattended by employees and without any physical barriers to protect against outside environmental factors or to prevent public access. Discussed an outdoor cooking approval is required. The person in charge ceased all outdoor cooking, removed all food equipment, and discarded the meat trompo at the time of inspection. Discussed gas tanks not be used or stored inside the establishment. Provided outdoor cooking approval resource information.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
