Addl Notes: Observed an employee with glove hands sweep then remove a fryer basket from the fryer and stop a timer without removing their gloves and washing their hands. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. The employee removed their gloves and washed their hands. PIC moved the fryer baskets to the dishwasher at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: No timestamp observed of TCS foods stored at room temperature including pizza sauce at 56*F and mozzarella at 52*F. PIC stated the foods were brought out at 1:50pm and are on time as a control. All TCS foods being held under time as a control were properly time stamped at time of inspection. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed containers of cooling diced chicken, tomatoes and spaghetti with internal temperatures ranging from 50-55*F in covered metal pans in the walk in cooler. All TCS foods cooling were uncovered to allow for rapid cooling. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Also observed already cooled chicken wings stored uncovered in the walk in cooler. Advised PIC to cover prepared foods to prevent contamination. PIC utilized plastic wrap to cover the cooled chicken wings at the time of inspection. Corrective Action: Corrected at time of inspection.

6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed 20+ drain flies and a live roach in the back warewashing and mop sink area. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection on 2/14/2025. Corrective Action: Correct prior to re-inspection.

4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Addl Notes: Observed a buildup of food debris and rusty wire shelving on a shelf in each walk in cooler. Observed cooling and uncovered TCS foods stored on racks before rusted shelving. All food-contact surfaces must be smooth, non-absorbent, and easily cleanable. Repair/replace and clean shelving prior to the next routine inspection. Corrective Action: Correct prior to next routine inspection.

PRIORITY VIOLATION-5-202.11 (A) - Priority: Approved System and Cleanable Fixtures; According to Law

Addl Notes: Observed a pressured hose and spray head stored inside the mop sink basin. PIC removed shut-off downstream from atmospheric vacuum breaker at time of inspection. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Corrective Action: Corrected at time of inspection.

Comments:

5-501.11 - Core: Outdoor Storage Surface Addl Notes: Observed grease receptacle stored behind the establishment on earth. An outdoor storage surface for refuse, recyclables, and returnables shall be constructed of nonabsorbent material such as concrete or asphalt and shall be SMOOTH, durable, and sloped to drain. Advised PIC to provide a concrete or similar material pad or relocate receptacles to another area prior to the next routine inspection. Corrective Action: Correct prior to next routine inspection.

Comments/Details

A re-inspection will be conducted on 2/14/2025 to ensure priority foundation violation #38 has been corrected.
No County legal action will result from this inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
