Addl Notes: Observed storage rack placed in front of hand wash sink blocking access to the hand wash sink by the ice machine. Observed inoperable hand wash sink by the ice machine and by the front counter. Hand sinks had no water supply and faucets were missing or unusable in state of disrepair. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Correct prior to re-inspection.

Observed organic matter on a soda nozzle of the soda fountain. Predefined Comment: All food-contact surfaces must be cleaned to sight and touch. Predefined Comment: All food-contact surfaces that contact TCS foods must be cleaned every 4 hours by washing, rinsing, and sanitizing. Predefined Comment: PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked rice with internal temperature of 109-120F stored in a pot by the steam table. The rice pot was stacked on top of other containers in the steam table and not in the hot water bath of the steam table. The person in charge stated the rice had been cooked less than 2 hours ago. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Predefined Comment: All TCS foods improperly hot held were reheated to 165°F at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed packaged foods from a recent delivery stored in the hallway by the walk-in cooler. Delivered TCS foods had internal temperatures between 59-67F and included raw meats and dairy products. The person in charge stated the food items had been delivered less than 4 hours ago. Observed unattended raw meat on the prep table and diced meat in a bucket next to the prep table with internal temperature of 69F. The person in charge stated the meat had been out less than 2 hours and was being prepared between downtime of other tasks. Predefined Comment: All TCS foods improperly cold held were relocated to refrigeration unit to rapidly cool to 41°F at time of inspection. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed no date marking system in use for any cooked TCS food items stored in multiple coolers in the establishment. No TCS food items had date marks. The person in charge was able to determine day of preparation for most TCS food items requiring date mark. Most TCS food items had been prepared between 2-6 days prior and were date marked accordingly. TCS food items with undetermined date of preparation were embargoed at the time of inspection. TCS foods with no date mark included cooked beans, cooked chile relleno, cooked fish, cooked shrimp, cooked beef, cooked pork, cooked chicken, cooked red/green salsas, cooked stews and broth, cooked meat rolled tacos, cut leafy greens, tomatoed pico de gallo, and deli ham. Predefined Comment: All RTE/TCS foods were properly date marked at the time of inspection. Predefined Comment: RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

No employee present at the time of inspection had a valid food handler certification. Employee who was working the cook line, doing food prep, and warewashing was unable to demonstrate food safety knowledge and provide correct answers regarding public health practices and principles applicable to the operation. Discussed proper cold/hot holding temperatures, reheating, date marking, and warewashing.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
