Addl Notes: Observed employee, after washing hands, handle the side of a trash can to move it and touched the lid to put it on the container, at the time of inspection. Employee was then going to make a residents drink. Employee stopped when prompted and went to rewash hands. Care shall be taken after hands are washed so that they are not recontaminated. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed two plates of beef tips and noodles and a plate of biscuit and gravy with internal temperatures between 123 and 127 degrees Fahrenheit in a hot box located in a servers pass through room. PIC had employee discard food at the time of inspection. TCS foods shall be held hot at 135 degrees Fahrenheit or above to prevent microbial growth. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed individual commercial packets of butter with internal temperature between 68 and 77 degrees Fahrenheit in a container at room temperature next to the server's juice and hot water dispensers. PIC discarded butter at the time of inspection as box said to keep butter refrigerated. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-7-204.11 - Priority: Sanitizers, Criteria-Chemicals
Addl Notes: Observed quaternary ammonia (quat) sanitizer with concentration above 500 ppm. PIC diluted sanitizer to 400 ppm at the time of inspection. Discussed diluting and checking each batch of sanitizer as dispenser is dispensing too strong. Also discussed following test strip instructions. Namely, immerse in the solution for 10 seconds, not in the foam and do not shake the test strip. For food contact surfaces, quat sanitizer shall be between 150 and 400 according to manufacturer guidelines. Corrective Action: Corrected at time of inspection.

3-501.13 - Core: Thawing Addl Notes: Observed frozen brussel sprouts on the counter in the prep area. Per employee, brussel sprouts and been there approximately an hour prior to inspection. Brussel sprouts were moved to be cooked promptly. All foods being thawed from frozen must be thawed in an approved manner. Foods must be thawed under refrigeration, under cold running water, or as part of the cooking process. Corrective Action: Corrected at time of inspection.

Properly calibrated probe thermometers are required at all times for monitoring food temperatures. Must provide a probe thermometer for establishment prior to re-inspection. Corrective Action: Correct prior to re-inspection.

3-305.11 - Core: Food Storage - Preventing Contamination from the Premises Addl Notes: Observed opened package of bread crumbs on the floor with items that had just been delivered. Ensure food deliveries are promptly moved at least 6 inches off the floor to prevent contamination from the premises. Corrective Action: Correct prior to next routine inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed ice cream scoops being stored in domestic use small crock pot warmer with water temperature of 130 degrees Fahrenheit. Employees washed scoops and replaced water from hot water dispenser. Water was 145 degrees when checked 30 minutes later. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed establishment using high temperature dishwasher without having a temperature measuring device. Discussed having 160 degree Fahrenheit dishwasher stickers, or a max/min thermometer for ensuring that at the point of the dishes, the temperature has reached at least 160 degrees Fahrenheit. Corrective Action: Correct prior to re-inspection.

Observed that sunkist juice products used in the dispenser do not specify on the package that juice is pasteurized. Establishment removes frozen juice concentrate containers from the shipping box for storage. Ensure juice product is a pasteurized product.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
