Addl Notes: Observed no paper towels or hand drying provision at the hand wash sink behind the bar. Predefined Comment: PIC provided hand drying provision for handwash sink at time of inspection. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed bottles and other containers stored inside the hand wash sink behind the bar. Predefined Comment: PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed no hand soap at the hand wash sink by the three compartment sink or at the hand wash sink by the reach-in freezer next to the expo line. Predefined Comment: PIC provided soap for handwash sink at time of inspection. Addl Notes: Observed dairy based whipped cream and dairy based half and half with internal temperature of 52F in the beverage cooler behind the bar. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. The person in charge was unable to determine how long the food items had been stored in the cooler, the food items were embargoed at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed the cooler behind the bar being used to store TCS foods (Time temperature control for safety) with minimum internal temperature of 52F. Establishments ambient thermometer inside the cooler displayed a temperature of 52F and inspector min/max thermometer showed minimum internal temperature of 53.4. The person in charge discontinued use of the cooler for cold holding and placed a work order for service at the time of inspection. Predefined Comment: Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct prior to re-inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed multiple ice scoops, used to service ice for consumption, stored with the food contact surface in direct contact with liquor bottles next the ice bin. Discussed food contact surfaces of utensils should not contact other unsanitized surfaces, only contact food, or be stored to protect from contamination. Predefined Comment: In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

MCEHC Chap. 8, Sec. 2, Reg. 5a - Latex gloves may not be used in direct contact with food. Addl Notes: Observed latex gloves at the bar used for handling and preparing ready to eat foods. The person in charge discontinued use of latex gloves and removed them from service to be replaced by approved non-latex gloves. Predefined Comment: Latex gloves are not approved for food handling. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed establishment utilize quat-sanitizer but have no quat-sanitizer test strips available. Predefined Comment: Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. 6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed an accumulation of soils on the cieling tiles and hood filters int eh back kitchen. Discussed cleaning frequency to prevent accumulation of grease, food pieces, dead insects and organic matter, under and around equipment on cook line, under storage racks and other areas of the establishment. Clean more frequently. Corrective Action: Correct prior to next routine inspection.

6-403.11 - Core: Designated Areas-Employee Accommodations for Eating/Drinking/Smoking Addl Notes: Observed empolyee drink containers stored directly on top of an in-use cutting board on the cook line. The person in charge removed the drink container and placed the cutting board for warewashing. Discussed storage of employee food/drink and personal items in designated areas away from frood prep, food storage, clean utensils, to prevent contamination. Corrective Action: Corrected at time of inspection.

Discussed keeping REDUCED OXYGEN PACKAGED (ROP) fish in its ROP packaging in the thawed state. Fish must be removed from ROP prior to thawing or immediately upon completion of thawing. Discussed prohibited animals in food establshement included outdoor patio. Provided dog friendly pation application information.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
