Addl Notes: Observed buildup of pink slimy organic matter inside tall ice machine. All food-contact surfaces must be cleaned to sight and touch. Addl Notes: Observed employee wash and rinse cups used for blending coffee beverages containing milk before being stored as clean without sanitizing in between. Per employee, cups had been in use since opening, more than 4 hours prior, and are normally sanitized at the middle/end of day. All food-contact surfaces that contact TCS foods must be cleaned every 4 hours by washing, rinsing, and sanitizing. Food-contact surfaces were properly sanitized at time of inspection. Corrective Action: Corrected at time of inspection.

3-501.17 - Priority Foundation: Ready-To-Eat Time/Temperature Control for Safety Food, Date Marking. Addl Notes: Observed two 12 quart containers of drink base made with dairy with a date indicating to discard on 5/23 (9 days post inspection). PIC indicated they go based off of the "best by" date printed on the original dairy container. PIC stated item was prepped the day prior and relabeled container for 5/13. Observed opened container of dairy vanilla base without a date. PIC stated container was opened 2 days prior and relabeled container. PIC was informed that any non-exempt ready to eat time/temperature controlled for safety (TCS) foods, whether they were made by a manufacturer or prepared in the establishment, must be sold or discarded within 7 days from open/preparation date (day 1 being the day they were opened or prepared). PIC was also informed that TCS foods must be dated within 24 hours, unless sold or discarded within that time. Corrective Action: Corrected at time of inspection. REPEAT VIOLATION (2ND CONSECUTIVE)

PRIORITY VIOLATION-7-204.11 - Priority: Sanitizers, Criteria-Chemicals
Addl Notes: Observed sanitizer bucket with wiping cloths in chlorine sanitizer with concentration above 200ppm. Per PIC, wiping cloths are used to wipe counters and occasionally, blenders/mixers. Sanitizer was diluted by PIC to approved levels at time of inspection. Discussed approved use of sanitizers with the person-in-charge (PIC). Sanitizer must be used at concentrations and in an approved manner per manufacturer instructions, 40 CFR 180.940, and 40 CFR 180.2020. Corrective Action: Corrected at time of inspection.

4-903.12 - Core: Prohibitions; Utensils, Equipment and Linens Addl Notes: Observed unopened box of straws stored on top shelf of toilet room. Also observed unpackaged beverage trays, clean linens, and cup sleeves stored in toilet room. Informed PIC that cleaned and sanitized equipment, utensils, laundered linens, and single service and single-use articles may not be stored in locker rooms; in toilet rooms; in garbage rooms; in mechanical rooms; under sewer lines that are not shielded to intercept potential drips; under leaking water lines including leaking automatic fire sprinkler heads or under lines on which water has condensed; under open stairwells; or under other sources of contamination. Informed PIC that restrooms could contaminate these single service/use articles. Person in charge indicated they would inform manager that items cannot be stored in restroom. REPEAT VIOLATION (4TH CONSECUTIVE)

Addl Notes: Observed chlorine test strips on-site with an expiration date in 2023. Test strips observed to be wet and discolored. Establishment observed to be using bleach/chlorine as sanitizer. Test strips are required for any sanitizers used with food contact surfaces. Provide chlorine sanitizer test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection.

No County legal action will result from this inspection.
Observed wet wiping cloths stored on counters. Discussed with PIC wet wiping cloths must be stored fully submerged in sanitizer buckets with an approved sanitizer concentration. Discussed storage of in-use utensils, such as dry ingredient scoops. Recommended replacing containers/cups with a container/cup with a handle.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
