Addl Notes: Observed alfredo sauce 84*F in the walk-in cooler with a time stamp to begin the cooling process at 12:50pm, three hours prior to the time of inspection. Cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. All TCS foods improperly cooled were discarded by PIC at time of inspection. Consecutive repeat violation: 3rd. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed marinara sauce at 128*F in hot holding unit by prep table. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Per PIC, marinara sauce had been reheated one hour prior and was hold held since. All TCS foods improperly hot held were reheated to 165°F at time of inspection. Consecutive repeat violation: 4th. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed provolone cheese 62*F, cheese sauce 49*F, cooked chicken 45*F-47*F, cooked pasta 47-54*F, ricotta cheese 52*F, butter with milk as ingredient 58*F, cooked peas 48*F, cut tomatoes 46*F in the first prep table on the cook line near hot holding unit . In the second prep table on the line observed broccolini 52*F, mozzarella 53*F, cut tomato 51*F, butter with milk as ingredient 51-53*F, clam 52*F, previously canned artichokes 51*F, alfredo sauce 49*F, raw shrimp 50*F, scallops 49*F, cooked pasta 48*F, clam juice 50*F. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. TCS food items held >4 hours above 41*F were discarded at the time of inspection. See embargo form. TCS food items held less than 4 hours above 41*F were moved to an alternative functioning refrigeration unit at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed the first prep table on the cookline hold TCS foods between 46*F-62*F. Inspectors min/max thermometer read 44.2*F and establishments dial thermometer in the unit read 43*F at the time of inspection. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Can be verified using electronic means. Corrective Action: Correct prior to re-inspection on 12/4/25.

Addl Notes: Observed vodka sauce 107-110*F cooling in the walk-in cooler in containers deeper than 4 inches at the time of inspection. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Per PIC and cooling time stamp, TCS vodka sauce started cooling one hour prior. All TCS foods being cooled in deep containers were transferred to shallow containers to allow for rapid cooling. Corrective Action: Corrected at time of inspection.

Description: Approved Thawing Methods Used

3-501.13 - Core: Thawing Addl Notes: Observed ROP fish marked to remove from packaging before thawing still in the package, thawed in a prep table at the time of inspection. All ROP fish labeled "Keep Frozen" must be removed completely from reduced oxygen packaging (ROP) prior to thawing under refrigeration or immediately after thawing under cold running water. All ROP fish was removed from ROP packaging at time of inspection. Corrective Action: Corrected at time of inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
