It appears an account has been created, but no mention of registration or payment towards the Food Protection Manager Exam. Inspector provided the phone number for the company. PIC wishes to complete the exam in Spanish. Corrective Action: Correct prior to next routine inspection. 4th repeat violation.

Addl Notes: Observed black liquid dripping on the sides of a Gourmet Ice unit with its contents including mango ice. Black mold was observed inside the opening of the dispenser area. PIC turned the unit off to clean the areas. Food contact surfaces must be cleaned daily. Corrective Action: Correct prior to next routine inspection. Addl Notes: Observed the following TCS food items holding 46-48F: mild salsa, cream, cooked corn, shredded cheese, chopped lettuce, pico, guacamole with chopped tomatoes, chopped cabbage, sour cream, and cooked potatoes. PIC stated that all products had been refilled into the table an hour prior. PIC replaced the out of temperature products with ones from the walk-in refrigerator that were holding 41F. PIC also stated that the pico and the corn were cooked/prepared today. Discussed cooling to 41F prior to placing them into the unit. The top portion of the prep table is designed to maintain cold temperatures. Please cool all food items in the walk-in refrigerator. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-7-207.11 (B) - Priority: Restriction and Storage-Medicine; Storage & labeling

Addl Notes: Observed a bottle of stomach relief in the walk-in refrigerator. The bottle was stored on the very top shelf next to sauces and above containers of salsa and cooked potatoes. Inspector asked employees if they were sick or had any symptoms of illness. Both employees said they were not sick. Employee discarded the bottle. Medicines and personal belongings must be stored in designated areas to prevent contamination. Corrective Action: Corrected at time of inspection.

4-301.11 - Priority Foundation: Cooling, Heating, and Holding Capacities-Equipment. Addl Notes: Observed the refrigerated assembly table holding 44.6F. The unit seemed to fluctuate between 42-48F during the inspection. All food products inside of the unit were no less than 44F. Employee told the Operator to call for maintenance and placed ice under the containers that are kept on the top portion. Corrective Action: Correct prior to re-inspection.

Emailed the Operator the weblink to sign up for the CFPM course in Spanish-as requested. Also note: please ensure all food
products come from approved sources. PIC stated that the flan is made at another location at 4825 E Warner Rd in Ahwatukee,
where there is an oven, but she the Bain-marie cooking process on top of the flat top grill per Operator. Discussed only selling the
item in store and requiring a food processing permit if needed. ***A re-inspection will occur by 7/26.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
