Addl Notes: Observed raw shelled eggs stored over containers of cut ready to eat vegetables (sliced cucumbers, diced onions, pico de gallo, chopped cilantro) in the prep cooler by steam table. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed build up of raw meat debris on the saw band, surfaces of the saw table, and other internal components of the meat saw stored in the back room next to the grease trap and mop sink. The person in charge stated the meat saw is used throughout the day and was unable to determine when last washed/rinsed/sanitized. Predefined Comment: All food-contact surfaces must be cleaned to sight and touch. Predefined Comment: All food-contact surfaces that contact TCS foods must be cleaned every 4 hours by washing, rinsing, and sanitizing. The person in charge discontinued use of the meat saw at the time of inspection. Corrective Action: Corrected at time of inspection.

3-305.11 - Core: Food Storage - Preventing Contamination from the Premises Addl Notes: Observed containers of cooked beans stored on the floor partially under 3comp sink between containers of cleaning chemicals and trash can. Observed employee warewashing and storing dirty dishes at the 3comp sink directly over the uncovered containers of cooked beans. Observed employee proceed to blending beans on the floor then store the containers in the walk-in cooler. Predefined Comment: Food must be stored at least 6 inches off the ground at all times to prevent contamination from the premises. Corrective Action: Corrected at time of inspection.
3-307.11 - Core: Miscellaneous Sources of Contamination Addl Notes: Observed open container of uncovered onions stored under the hand wash sink and next to a trash can. Observed in-use meat saw table in the back room next an open ground trench around the grease trap and piles of soil from ongoing construction. Discussed food and food equipment to be protected from sources of contamination. The person in charge properly stored the container of onions and discontinued use of the meat saw at the time of inspection. Corrective Action: Corrected at time of inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed in-use tongs stored between use in the trash can next to the 3comp sink and trash can next to the cook line. Observed food contact surface of tongs in contact waste inside trash cans. Observed ladle used to stir chicharron in fryer stored hanging from fire extinguisher and in contact with soiled wall covering. Predefined Comment: In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

The person in charge stated waste water from the mop is poured in trash bags and discarded in the dumpster. Addl Notes: Observed open trench around underground grease trap with broken/unsealed concrete flooring and piles of soil in the back mop room. Grease traps shall be designed, constructed, and installed according to law. Discussed improper repair of maintenance of any portion of the plumbing system may result in potential health hazards, such as cross contamination, backflow, or leakage. These conditions may result in the contaminations of food, equipment, utensils, linens or single service articles. It may contribute to obnoxious odors or nuisances and can affect the operation of warewashing equipment or other equipment that depends on sufficient volume and pressure to perform intended functions. 6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed accumulation of soils, food debris, and other organic matter on the door handle of the walk-in, around the threshold and door frame of the walk-in cooler, on the floor/walls/ceiling of the walk-in cooler. Observed accumulation of dust soils and grease residues on the filters of the ventilation hood, fire suppressant lines, and walls around the ventilation hood. Observed accumulation of soils on the ceiling in the area directly above the outdoor grill. Predefined Comment: Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: Observed accumulation of broken unused equipment and other rubbish in the outdoor enclosure behind establishment. Observed power drill stored on sheet pan in the speed rack next to the walk-in cooler. Discussed removal of litter and articles that are not needed for the operation or maintenance of the establishment. Corrective Action: Correct prior to next routine inspection.

The light intensity shall be at least 108 lux (10 foot candles) at a distance of 75 cm (30 inches) above the floor, in walk-in refrigeration units. "A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
