At time of inspection, no certified food protection manager (CFPM) was present on the premises. A CFPM must be present at all times of operation. Please have a CFPM present for the next inspection. Addl Notes: Observed employee place raw chicken from the prep counter into a container. Employee placed the container in the walk in cooler, changed their gloves, and then proceeded to go handle ready to eat rice without washing their hands. A full 20 second hand wash is required between raw and ready to eat foods. Employee was instructed to wash their hands and dawn new gloved before handling the rice. Corrective Action: Corrected at time of inspection.

Addl Notes: On the steam table in the front service counter, observed chow mein at 130*F, fried rice at 125*F, cooked veggies with chicken at 118*F, cooked rice noodles at 123*F, cooked lemon chicken at 125*F, cooked orange chicken at 108*F, cooked broccoli beef at 110*F, cooked mushroom chicken at 115*F, pepper beef at 109*F, BBQ chicken at 115*F, kung pao chicken at 110*F, BBQ pork between 104*F and 125*F, spring rolls at 107*F. Steam table was turned up. All foods were placed in the unit approximately an hour prior. All foods were reheated and verified with inspector's probe thermometer to 165*F or higher. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Corrective Action: Corrected at time of inspection.

Addl Notes: In the prep cooler by the wok station, observed raw bean sprouts at 47*F, raw beef at 47*F, and raw chicken at 45*F. All foods were placed in the unit approximately an hour prior. Foods were moved to the walk in cooler to rapidly cool. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed a bottle of supplements and a bottle of prescription medication stored on the shelf directly above a prep table in use for raw chicken. Predefined Comment: All medicines must be stored away from food, utensils, and single service items to prevent contamination. All medicines were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Comments:

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed food debris accumulating on the floors and wall around the cooking equipment. Observed food debris and residues accumulating on the floors and shelving of the walk in cooler. Observed dust accumulating on the ceiling around the air conditioning vents. Please have facility cleaned more frequently. Facility should be cleaned as frequently as necessary to prevent soils, debris, and residues. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
