Addl Notes: Observed container of raw pork stored over cooked ready to eat foods in the walk-in cooler. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed containers of asiento (rendered pork lard with cooked pork particulate) and chilorio (cooked pork in dry chile sauce) with internal temperature of 79F displayed for sale on top of the meat counter. The person in charge stated the asiento and chilorio is cooked and prepared on site. Discussed both food items as TCS requiring date marking and time/temperature control for safety. The person in charge was unable to determine when the food items had been removed from temperature control and placed at ambient temperature. Asiento and chilorio were embargoed at the time of inspection. Predefined Comment: All TCS foods improperly cold held were discarded by PIC at time of inspection. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed establishment utilize chlorine sanitizer but portioned by hand for warewashing at the 3comp sink but have no chlorine sanitizer test strips available to test sanitizer concentrations. The person in charge obtained sanitizer test strips at the time of inspection. Predefined Comment: Test strips are required for any sanitizers used with food contact surfaces. Corrective Action: Corrected at time of inspection.

4-601.11(B) and (C) - Core: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils Addl Notes: Observed build up of food residues on the control panel of the microwave and food debris inside the microwave. Observed accumulation of food debris on food containers/plates in a crate next to the cook line. Predefined Comment: Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Corrected at time of inspection.

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed accumulation of food splatter/residues on the back wall of the cook line and on the wall under the microwave. Observed accumulation of food debris on the floor under and around the steam tables. Observed accumulation of dust soils on the ceiling tiles around the vents over the cook line. Predefined Comment: Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
6-501.11 - Core: Repairing -Premises, Structures, Attachments, and Fixtures-Methods Addl Notes: Observed gaps/holes in ceiling with broken/missing ceiling tiles by the dry storage racks, over the steam tables, and other areas of establishment. Observed holes in wall around plumbing penetrations under 3comp sink. Observed unsealed flooring and broken floor tiles by the steam table and in the walk in cooler. Observed outer opening from gap along the bottom of the backdoor threshold. Predefined Comment: All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Predefined Comment: Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed mixed date marking system with some cooked ready to eat TCS date marked with date of preparation and others date marked with date of expiration. Discussed utilizing a single date marking system to verify actual date of disposition.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
