Addl Notes: Observed cooling watermelon at 55*F and rice noodles placed in room temperature water measuring 71-74*F in the open top cooler in the smoothie area. Per PIC, the foods had been prepared at opening less than 2 hours prior. Observed other foods in the unit holding at 41*F or below. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Discussed that this cooler is not intended to cool TCS foods. Establishment does not have an NSF cooler intended to cool TCS foods. PIC added ice to the rice noodles and stated the watermelon would be processed and frozen ASAP. Corrective Action: Corrected at time of inspection.

Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed in use utensils stored in still water measuring 99*F when measured with a probe thermometer. Observed In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

4-101.11 (B-E) - Core: Characteristics- Multiuse; Durable, Nonabsorbent, Smooth, Easily Cleanable Addl Notes: Establishment does not have an NSF cooler capable of cooling TCS foods. Establishment is using a residential styler cooler and a NSF #7 that is not intended to cool TCS foods as their primary coolers. Establishment must purchase a NSF or equivalent unit that is capable of cooling TCS foods prior to the next routine inspection. Advised PIC to limit menu to limit cooling requirements. Corrective Action: Correct prior to next routine inspection.
4-101.19 - Core: Nonfood-Contact Surfaces Addl Notes: Observed loose tiles on the countertop adjacent from the soda machine. All nonfood-contact surfaces must be smooth, non-absorbent, and easily cleanable. Repair/replace tiles prior to the next routine inspection. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-5-203.14 - Priority: Backflow Prevention Device, When Required
Addl Notes: Observed a RO (reverse osmosis) water system connected below the hand sink without proper backflow. All RO systems must have an approved reduced pressure zone assembly backflow device meeting ASSE 1013 standards installed upstream from RO system. Discussed the acidifying elements may be removed (so only the carbon filters remain) or the system may be removed from the establishment. PIC stated the system is not being used at this time. Advised PIC to fully disconnect the filter. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
