Addl Notes: On cook line, employee touched clothing several times with gloved hands then continued working with food equipment and ready to eat foods without a proper handwash. Instructed employee to remove gloves, wash hands, and don new gloves to continue with task. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Wash hands at any time contamination occurs and when changing tasks. Corrective Action: Corrected at time of inspection.

Addl Notes: In prep area, slicer with accumulation of food debris on blade and nonfood contact surfaces. Per manager, slicer was not used today and stored as clean. Instructed employee to re-wash, rinse, and sanitize slicer. Discussed more frequent monitoring of food contact surfaces after warewashing and before storing. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Corrected at time of inspection.

Addl Notes: Third Consecutive Violation. On cook line, in two drawer cooler, TCS foods (raw fish, cooked chicken wings) with internal temperatures of 50*F-52*F. Per manager, raw fish and cooked wings were in prep table cooler overnight. Raw fish and cooked chicken wings were discarded. See embargo form. Ambient temperature of drawer cooler was 48.2*F, 47.9*F, and 48.2*F, taken with inspector min/max thermometer. Ambient temperature of 43*F with unit digital thermometer. Discontinue use of two drawer cooler for storing of TCS foods until able to maintain proper temperatures throughout the day. Recommended operator to call repair company. Maintain TCS (time/ temperature control for food safety) foods at a cold holding temperature of 41*F or less at all times. Corrective Action: Corrected at time of inspection.

Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and SANITIZED or discarded if they are not capable of being resurfaced. Corrective Action: Correct prior to next routine inspection.

4-602.13 - Core: Nonfood Contact Surfaces Addl Notes: Throughout kitchen, on cook line, inside of prep table coolers, doors and handles, and in prep area, shelving and nonfood contact surfaces of mixers and blenders with accumulation of food residues. Maintain clean. Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

Reviewed with manager and employees: 7-day discard date, preparation times out of cold holding.
Recommended obtaining a bodily fluids clean up kit.
Inspection conducted with RS#874.
Ensure all employees working with food obtain food worker certification from ANSI certified online site.
Routine inspection report was emailed to person in charge at establishment.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
