Addl Notes: Observed no paper towels at the hand wash sink by the expo line behind the bar. Predefined Comment: PIC provided hand drying provision for handwash sink at time of inspection. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-4-501.114 (A-E) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Addl Notes: Observed wet dishes in the mechanical dishwasher at the bar with 0ppm chlorine sanitizer. Observed an empty chlorine sanitizer container connected to the mechanical dishwasher. The person in charge replaced the empty sanitizer container with a full container at the time of inspection. Predefined Comment: Food-contact surfaces must be properly sanitized after washing and rinsing. Predefined Comment: Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Predefined Comment: All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-3-501.16(A)(2) and (B) - Priority: Time/Temperature Control for Safety Food, Cold Holding. Addl Notes: 2nd consecutive violation. Observed TCS foods with internal temperatures between 44-51F stored in multiple prep coolers on the cook line. TCS food items included sliced deli meats (pork, turkey, corned beef brisket), various soft sliced cheeses, packaged raw burger patties, packaged steaks, raw fish, and cut cabbage salad. The person in charge stated all food items had been stored in the respective prep coolers since the day before, more than 4 hours. All TCS food items not properly cold holding at 41F or less were embargoed at the time of inspection. Predefined Comment: All TCS foods improperly cold held were discarded by PIC at time of inspection. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed multiple cold holding units with minimum internal ambient temperatures between 46-53F all located along the cook line. Observed cooler under the grill top with minimum internal temperature of 51F, a prep cooler by the steam table with minimum internal temperature of 53F, and a prep cooler on the back wall with minimum internal temperature of 46F. Observed drawers and doors of the cold holding units in a state of disrepair. Predefined Comment: Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct prior to re-inspection.

Description: In-Use Utensils; Properly Stored

Comments:

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed ice scoop used to serve ice for consumption stored wedged between liquor bottles with the food contact surface of the ice scoop in direct contact with unsanitized surfaces of the liquor bottles. Discussed proper storage of utensils. Predefined Comment: In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

Description: Plumbing Installed; Proper Backflow Devices

Comments:

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed a bucket with waste water and loose plumbing connections under the three compartment sink. The person in charge stated the bucket is used to catch a leak from the center compartment of the three compartment around the drain. Predefined Comment: Must repair all plumbing to a state of good repair prior to next routine inspection. Predefined Comment: All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to next routine inspection.

Description: Dog Friendly Patio Requirements (FD)

Comments:

MCEHC Chap. 8, Sec. 2, Reg 4(b) - A sign with at least half inch letters shall be posted at the front entrance of the food establishment so that it is easily visible to the public. The sign shall state: "Dog Friendly Patio - Dog access only through outdoor patio. For violations, contact Maricopa County Environmental Services Department (602) 506- 6616." Addl Notes: Observed no dog friendly patio signage at any of the patio gate entrances. A sign with at least 2 inch letters shall be posted at the front entrance of the Food Establishment so that it is easily visible to the public. The sign shall state: “Dog Friendly Patio - Dog Access Only through Outdoor Patio. For violations, contact MCESD (602) 506-6616.” Corrective Action: Correct prior to next routine inspection.

MCEHC Chap. 8, Sec. 2, Reg 4(d) [4] - All dog waste shall be placed in a fly-tight container located adjacent to the patio area and disposed of outside of the food establishment in an appropriately covered waste receptacle. Equipment used to clean the outdoor patio must be kept outside of the food establishment. Addl Notes: Observed no fly tight covered receptacle for dog waste in the vicinity of the dog friendly patio. All dog waste shall be placed in a fly-tight container located adjacent to the patio area and disposed of outside of the Food Establishment in an appropriately covered waste receptacle. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
