Addl Notes: Observed employee handle cash at the register and then put on gloves to start cooking without washing hands. Discussed with Person in Charge (PIC) discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Employee properly washed hands at handwash sink at time of inspection. Co rrective Action: Corrected at time of inspection.

Addl Notes: Observed several RTE/TCS items stored in the walk-in and the reach-in without any date marking. Per Person in Charge (PIC) the items were made on Monday. The items were baba ghanoush, sliced tomato, yogurt sauces (pink and white), cooked steak, falafel, and gyro meat. PIC date-marked all items at time of inspection. Corrective Action: Corrected at time of inspection.

3-602.11 (A)(B1-4)(B6-7)(C)(D) - Core: Food Labels Addl Notes: Observed labels for the baklava in the facility but not on the portioned containers of baklava that were made from a different permitted facility. The labels state in English the name of the company, the ingredients, and that the product is baklava. The labels are missing the address or phone number of the company and approximate weight of each item. Discussed with PIC Labeling shall contain 1) Name of Product; 2) Processor Name; 3) Processor Address or Phone Number; 4) Ingredients; 5) Approximate Weight; 6) In English. Corrective Action: Correct prior to next routine inspection. 3rd Consecutive Violation.

4-402.12 - Core: Fixed Equipment, Elevation or Sealing Addl Notes: Observed separate caulking and black organic matter along the wall of all three compartment sink. Discussed with PIC that fixed equipment that is not easily moved to clean around and behind shall be sealed with caulking to create a smooth and cleanable surface. Per PIC that will be fixed immediately. Corrective Action: Correct Prior to Next Routine Inspection. 2nd Consecutive Violation

PRIORITY VIOLATION-5-202.13 - Priority: Backflow Prevention, Air Gap

Addl Notes: Observed the rinse sprayer arm not in use hanging below the flood rim of the three-compartment sink. Discussed with PIC a minimum of 1 -inch of air as a gap between the sprayer arm and the flood rim shall be present to prevent contamination as there is no backflow device on this equipment. Discussed with PIC must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to re-inspection on 6/3/25

Comments:

6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed build up of grease, dust and organic matter on the walls in the kitchen, in the dishwasher area, the reach-in shelving. Discussed with Pic physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Of Note: Discussed with PIC 1) Single use containers such as large yogurt buckets are not designed to be washed and reused. 2) if storing scoops in the seasonings and rice they shall have a handle and handle shall be stored upright.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
