Addl Notes: Observed cook dredging raw chicken with gloved hands at time of inspection. After handling the raw chicken observed the chef wash their hands for ~5 seconds. Predefined Comment: Discussed the importance of proper hand washing procedures with the person-in-charge (PIC). Proper handwashing procedure is to rinse hands with warm water, scrub with soap for 10-15 seconds, rinse with warm water, dry with approved drying provision, and turn off in a manner to prevent re-contamination of hands. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination. Predefined Comment: Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Predefined Comment: PIC relocated all items blocking handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed a small covered pan of raw chicken stored directly above raw unpasteurized shelled eggs in the large two door reach in refrigerator in the prep area. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed low temperature dish machine being used to sanitize pans and a whisk at time of inspection. Final rinse chlorine sanitizer measured at 0 PPM. Final rinse chlorine sanitizer concentration shall measure between 50-100 PPM. Instructed staff to sanitize all food contact surfaces in the three compartment sink until the dish machine is serviced and maintains chlorine sanitizer concentration between 50-100 PPM. Predefined Comment: Food-contact surfaces must be properly sanitized after washing and rinsing. Corrective Action: Corrected at time of inspection.

Addl Notes: In the prep refrigerators on the line and drawer refrigerator below the flat top grill observed roasted corn/57°F, blue cheese/52°F, chopped lettuce/ 56°F, French toast batter/46°F, yogurt/44°F, sliced tomatoes/45-55°F, brie cheese/46°F, feta cheese/47°F, sliced turkey/ 54°F, cooked duck/48-54°F, sliced ham/52°F, cooked steak/52°F, chopped cabbage/56°F, goat cheese/49°F, cooked sweet potato/50°F, cooked salmon/52°F, cheddar cheese/58°F, cooked potatoes/55°F, and cooked fries/62°F. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below After discussion with the person in charge it was determined all TCS foods were placed in the units with morning around 8 AM. Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Corrective Action: Corrected at time of inspection. Repeat Violation: 3rd.

Addl Notes: Observed hollandaise sauce/112°F stored near the expo station at room temperature. Hollandaise sauce held using 4 hour TPHC instead of temperature control. No discard time observed for the hollandaise sauce. Predefined Comment: Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. After discussion with the PIC it was determined the hollandaise was prepared at 8AM. Predefined Comment: All TCS foods being held under time as a control were properly time stamped at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed thermometer on the drawer refrigerator below the grill displaying 63°F. Minimum temperature of three prep refrigerators measured 50.2°F, 62.8°F, 56.7°F at time of inspection with min/max thermometer. Predefined Comment: Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct within 24 hours.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
