Discussed the importance of preventing bare hand contact with ready-to-eat (RTE) foods with the person-in-charge (PIC). Bare hand contact is not approved with ready-to-eat (RTE) food items. Addl Notes: Second Consecutive Violation. On cook line, on cook top, on shelf above flat top, cooked hash browns with internal temperatures of 110*F-125*F. Per manager, hash browns were prepared within normal time parameters. Manager reheated hash browns to 165*F. Discussed the importance of maintaining proper hot holding temperatures. Maintain TCS (time/ temperature control for food safety) foods at a hot holding temperature of 135*F or higher at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: Second Consecutive Violation. On cook line, in cold holding drawers under cook top, TCS foods (raw proteins, cooked vegetables) with internal temperatures of 45*F-50*F. Per employees, TCS foods were in cold holding drawers within normal time parameters. Raw proteins, cooked vegetables were moved to walk in cooler to rapidly cool back down to proper temperature. Discussed new gaskets around cold holding drawers to ensure proper seal. At side prep area, in containers sitting on ice, TCS foods (cooked vegetables, cooked sausages) with internal temperatures of 55*F. Per employee, TCS foods were removed from cold holding <2 hours. Employee moved cooked vegetables and cooked sausages to cold holding drawer in prep table cooler to rapidly cool back down to the proper temperature. Discussed the use of an ice bath instead of containers sitting on ice. In employee cafeteria, at salad bar, TCS foods (cut lettuce, cut tomatoes, cut deli ham) with internal temperatures of 50*F-60*F. Per employee, salad bar items were removed from cold holding about one hour. Instructed employees to provide lids, ice baths, or move all TCS foods to reach in refrigerator to rapidly cool back down to the proper temperature. Ensure salad bar cold holding unit is able to maintain proper temperature before storing of TCS foods. Maintain TCS (time/ temperature control for food safety) foods at a cold holding temperature of 41*F or less at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: In low reach in cooler, couscous/ 8-12, in walk in cooler, couscous/ 8-12 and cooked duck/ 8-14 and tomato mixture/ unknown preparation date past 7-day discard dates. Couscous, cooked duck, and tomato mixture were discarded. See embargo form. RTE/TCS foods requiring date marking are to be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

3-501.15 (A) - Priority Foundation: Cooling Methods. Addl Notes: On speed rack in kitchen, cooked chicken fried steak/ 75*F prepared and cooling at room temperature about 1 hour. Employee moved sheet pans of chicken fried steak to walk in cooler to continue the cooling process. In walk in cooler, cream based sauce/ 60*F, salsa/ 65*F, egg salad/ 55*F prepared <2 hours and cooling in covered containers >4inches deep. Cream based sauce, salsa, egg salad were portioned in to uncovered shallow containers and moved back in to walk in cooler to continue the cooling process. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Corrective Action: Corrected at time of inspection.

4-302.13 - Priority Foundation: Temperature Measuring Devices, Manual and Mechanical Warewashing. Addl Notes: Establishment uses a high temperature warewashing machine for sanitizing of dishware. Establishment does not have a high temperature measuring thermometer for measuring final rinse of machine. Sanitizing using high temperature warewashing machines requires an irreversible measuring device to ensure food contact surfaces reach 160°F. Provide approved irreversible measuring device prior to re-inspection. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
