Addl Notes: Observed raw calamari and raw eggs stored above ready-to-eat foods in the coolers on the cook line. All raw animal proteins must be stored below ready-to-eat foods to prevent contamination. Raw calamari and eggs were moved below at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked mussels in the reach-in table on the cook line with an internal temperature of 56*F when measured with a probe thermometer. According to cook, mussels had been in cooler for two days. Observed cooked chicken in the bottom of the reach-in with an internal temperature of 47*F when measured with a probe thermometer. According to cook, chicken had been in cooler overnight. All TCS (time and temperature controlled for safety) foods must maintain internal temperatures of 41*F and below to prevent foodborne illness. Mussels and chicken were discarded at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed several trays of raw sushi fish in the sushi display case with internal temperatures of 46*F-50*F when measured with a probe thermometer. According to sushi chef, fish was prepped 30-45 minutes prior and placed in display case. All prepped TCS foods must be cooled to 41*F in the walk-in or freezer before being placed in display cases or cold holding units. Sushi display cases do not have the proper capacity to cool TCS foods when they are out of temperature. Fish was moved to walk-in at time of inspection to properly cool. Corrective Action: Corrected at time of inspection.

6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed one large live roach in the back of house, several dead roaches in the back of house, and one dead roach at the sushi bar area. Premises must be kept free of roaches and pests to prevent contamination of food and food contact surfaces. A re-inspection will be conducted on 4/25/25.

Addl Notes: Observed hand sink next to dish pit with no hot or cold running water. According to employee, hand sink has been turned off because of a leak. All hand sinks must have running water to ensure proper hand washing is taking place, and all plumbing must be maintained in good condition. Please repair and turn on water. A re-inspection will be conducted on 4/18/25. Corrective Action: Correct prior to re-inspection.

Please deep clean and maintain clean to eliminate pest harborage conditions. Corrective Action: Correct prior to next routine inspection.

Inspection report delivered via email. A re-inspection for dish pit hand sink will be conducted on 4/18/25, and a re-inspection for parasite destruction forms and presence of roaches will be conducted on 4/25/25.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
