Addl Notes: Observed employee wearing gloves drop a wiping cloth on the ground and pick wiping cloth up. Without changing gloves employee continued to grab a bowl and a plate/tray for a customer and fill the tray with fries and chicken fingers. Also observed employee on dish line washing dirty dishes, handling dirty dishes, and then immediately pull a clean tray out of the dish machine to dry. Discussed with PIC who educated employees about properly washing hands when they have become contaminated and prior to handling food or clean dishes. Corrective Action: Corrected at time of inspection.

Addl Notes: In back kitchen prep area near reach-in freezer the hand sink did not have paper towels. PIC provided and will maintain at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: In reach-in refrigerator across from gill line observed an open container of raw chicken being stored directly over a squirt bottle and a larger container of ranch dressing. Chicken was moved during inspection. PIC will ensure that all raw foods are properly stored to prevent contamination. Discussed proper food storage with PIC. Corrective Action: Corrected at time of inspection.

Addl Notes: In back kitchen prep area observed a dicer with food debris between the cutting surfaces. The dicer was being stored as clean. Also observed an industrial can opener with food debris on the surface. PIC will ensure that all equipment is properly cleaned and sanitized prior to being stored as clean. Dicer and can opener were moved to dish area to be properly cleaned. Corrective Action: Corrected at time of inspection.

Addl Notes: In reach-in refrigerator on server line observed an opened container of milk that was dated 8/9. PIC confirmed this was the date milk was opened and voluntarily discarded milk. Ensure that all TCS foods are only kept for 7 days. 2nd occurrence. Corrective Action: Corrected at time of inspection.

4-501.11 - Core: Good Repair and Proper Adjustment-Equipment Addl Notes: Reach-in freezer in kitchen prep area has a heavy water leak inside. Condenser is completely frozen over and water has dripped onto shelving and packaged foods, and then frozen. Please repair unit so that it is in good working order and move food items so that they are not under the leaking condenser. Corrective Action: Correct prior to next routine inspection.

Parts are beginning to come up from the ground and water is beginning to pool. Please correct so that floor is smooth, durable, and easily cleanable. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
