Addl Notes: SECOND OCCURRENCE Hand sink near walk-in refrigerator on kitchen prep line was missing paper towels. Also, bar hand sink was missing paper towels. PIC provided during inspection. Corrective Action: Corrected at time of inspection. Predefined Comment: Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing.

Addl Notes: SECOND OCCURRENCE In reach-in refrigerator across from fryer inspector observed a container of corn dog batter (with raw shelled eggs) being stored above raw fish. In walk-in refrigerator inspector observed a prep tray with raw shelled eggs and raw beef being stored directly above a large container of celery. In walk-in, inspector observed a working container of beer being stored below raw beef patties. In walk-in refrigerator inspector observed pans of raw beef burgers and raw chicken being stored above focaccia and pizza dough. PIC re-arranged all items during inspection so that cross contamination would not occur. Corrective Action: Corrected at time of inspection. Predefined Comment: All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code.

Addl Notes: Chlorine dish machine was in-use during inspection. Inspector observed a chlorine concentration of 0ppm while dishes were being washed. The supply bucket of chlorine was empty and PIC replaced with a new supply bucket. All dishes were re-washed once the dish machine reached a chlorine concentration of 50 ppm (must be between 50-100 ppm). Corrective Action: Corrected at time of inspection. Predefined Comment: Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Predefined Comment: Food-contact surfaces sanitized using chlorine sanitizer must be sanitized with a contact time of least 10 seconds.

Addl Notes: SECOND OCCURRENCE On back kitchen prep line inspector observed an industrial canner, two dicers, and a slicer with food debris on the surfaces. PIC stated that items had not been used the day of inspection. Corrective Action: Corrected at time of inspection. Predefined Comment: All food-contact surfaces must be cleaned to sight and touch. Predefined Comment: PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection.

Addl Notes: THIRD OCCURRENCE In walk-in refrigerator inspector observed a container of cooked mushrooms dated 11/26. Corrective Action: Corrected at time of inspection. Predefined Comment: All RTE/TCS foods past 7 days of preparation were discarded by PIC at the time of inspection. Predefined Comment: RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1).

Addl Notes: Near dish machine inspector observed a spray nozzle hanging below the rim of the sink. No backflow preventer was present. PIC moved spray nozzle to hook that was on the spray nozzle line so that a 1 inch air gap was present. Maintain at all times when not in-use. Corrective Action: Corrected at time of inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
