Addl Notes: Observed hot water faucet to be in disrepair at the handsink in the sushi station. Repair the faucet to ensure hot water is also able to be dispensed from the hand sink. Addl Notes: Observed packaged ready to eat shrimp stored below raw salmon and raw tuna on the storage rack in the walk-in cooler. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection. 2nd Repeat Violation.

Addl Notes: Observed organic matter on the beverage dispensing nozzle in the bar area. Ensure food contact surfaces with non-TCS foods are cleaned at a frequency of at least 24 hrs to remove accumulation. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed cutting boards and knives used to cut raw fish at the sushi station to be cleaned using only a sanitizer towel and cleaned once daily as stated by the person in charge. At the time of inspection it had been past 4 hours of use and only cleaned with a sanitizer towel. All food-contact surfaces that contact TCS foods must be cleaned every 4 hours by washing, rinsing, and sanitizing. The cutting boards and knife were removed and taken to the back and properly washed rinsed and sanitized. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed the can opener with soiled old food residue on the piercing blade and placed in the holder as clean on the prep table. PIC stated the can opener was clean. The employee then took the can opener outside to the mop sink and was starting to clean the can opener. The PIC was instructed to have the employee take the can opener to the 3- compartment sink to be properly cleaned and sanitized. Education was provided regarding proper cleaning and sanitizing and proper use of equipment to clean and sanitize. Food-contact surfaces were properly sanitized at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed chopped cabbage measured at 47F in the reach in cooler across from the cooks line. The person in charge indicated the food was in the unit for 1 hour. Predefined Comment: All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Observed squid salad measured at 46F and two containers of crab mixture to measure between 49F and 52F in the reach in cooler at the sushi bar. The person in charge indicated the foods were in the reach in cooler over 4 hours. All TCS foods improperly cold held were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection. 2nd Repeat Violation

Observe employee food stored next to food used for restaurant operations in the walk- in cooler. Ensure employee items are stored in designated areas to prevent contamination. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
