Comments:

PRIORITY VIOLATION-4-501.114 (F1-2) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- On-site Generation. Addl Notes: Observed chemical dishwasher in bar with a chlorine concentration of 0 ppm chlorine at the time of inspection. Establishment was actively utilizing bar dish machine during inspection. Person in charge (PIC) primed dishwasher and ran the machine additional times; no chlorine was observed from any of the runs. Establishment instructed not to use dish washer until it has a measure chlorine concentration of 50-100 ppm. Establishment agreed to use main dish machine in the kitchen back of house in meantime. Discussed the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked chicken wings, raw steaks, cooked pork belly, and raw chicken, and cooked asparagus in a drawer refrigerator unit below the grill with temperatures between 46 to 54*F at time of inspection. Observed ambient temperature of the cooler at 51*F at time of inspection. Per cook, all items were placed in drawer unit less than 3 hours prior to inspection and were moved to the walk-in to rapid cool at time of inspection. Observed milk, whipped cream, & half & half in reach in cooler at the bar with temperatures between 45 to 50*F at time of inspection. Observed ambient temperature of cooler at 47.8*F at time of inspection. Per employee, milk was placed in reach-in cooler the day prior to inspection and embargoed at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. All TCS foods improperly cold held were discarded by PIC at time of inspection. Repeat violation: 4th Consecutive. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed multiple menu items (steak, burgers, eggs cooked to order) on the breakfast, brunch, lunch, and dinner menu that are offered undercooked without the disclaimer (asterisk) on the menu at time of inspection. Offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Must provide approved consumer advisory at all points of sale prior to re-inspection. Provided PIC with consumer advisory guidance document at time of inspection. Corrective Action: Correct prior to re-inspection on 7/3/2025.

7-202.11 - Priority Foundation: Restriction-Presence and Use Addl Notes: Observed aerosol can of roach killer in the chemical area in the kitchen at time of inspection. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed reach-in cooler holding milk for the coffee station at the bar holding multiple TCS items (See #22) with an ambient temperature of 48.7*F at time of inspection. Observed refrigerated drawer unit holding multiple TCS food items (see #22) with an ambient temperature of 51*F at time of inspection. Predefined Comment: Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct prior to re-inspection on 7/3/2025.

Due to the violations noted on this inspection report, County legal action may result from this inspection.
Observed handwashing sink in the main dish area moved to opposite wall in the dish area at time of inspection. Per chef, establishment plans to install a larger dish machine at time of inspection. Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 5 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
