Addl Notes: Second Consecutive Violation. In kitchen, employee handling cooking utensils and preparing food after contamination on hands. Employee touched clothing, dirty dishware then continued handling cooking utensils without a proper handwash. Instructed employee on proper handwashing before donning gloves to handle food products. Discussed the importance of proper handwashing with employee. FOOD EMPLOYEES shall clean their hands and exposed portions of their arms as specified under § 2-301.12 immediately before engaging in FOOD preparation including working with exposed FOOD, clean EQUIPMENT and UTENSILS, and unwrapped SINGLE-SERVICE and SINGLE-USE ARTICLES and: (A) After touching bare human body parts other than clean hands and clean, exposed portions of arms; (B) After using the toilet room; (C) After caring for or handling SERVICE ANIMALS or aquatic animals as specified in ¶ 2-403.11(B); (D) Except as specified in ¶ 2-401.11(B), after coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating, or drinking; (E) After handling soiled EQUIPMENT or UTENSILS; (F) During FOOD preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks; (G) When switching between working with raw FOOD and working with READY-TO-EAT FOOD; (H) Before donning gloves to initiate a task that involves working with FOOD; and (I) After engaging in other activities that contaminate the hands. Addl Notes: On cook line, in prep table cooler, TCS foods (dahl, yogurt, cooked sauces, hard boiled eggs, cooked chicken, cooked potatoes) with internal temperatures of 55*F-62*F. Per employee, TCS foods were in prep table cooler overnight. TCS foods (dahl, yogurt, cooked sauces, hard boiled eggs, cooked chicken, cooked potatoes) were discarded. See embargo form. Ambient temperature of prep table cooler was 59.8*F, taken with inspector min/max irreversible thermometer. Discontinue storing of TCS foods in prep table cooler until able to maintain proper temperature throughout the day, especially during busier times of service. Per person in charge, repair company is scheduled for site visit later on in day. Maintain TCS (time/ temperature control for food safety) foods at a cold holding temperature of 41*F or less at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: Third Consecutive Violation. In walk in cooler, cooked proteins (goat and chicken), cooked sauces with vegetables, samosas with vegetables, without proper date marks. Per person in charge, TCS foods were prepared >24 hours. Instructed employee to provide date marks to TCS foods (cooked proteins (goat, chicken), cooked sauces with vegetables, samosas with vegetables). Discussed proper date marking, with preparation date as DAY 1 and 6 additional days for a total of 7 days. Corrective Action: Corrected at time of inspection.

2) Remove curtain and curtain rod in-between front service area and main cookline kitchen area, to provide unobstructed access to hand sink. Otherwise, a hand sink must be installed in main cookline kitchen area.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
