Addl Notes: Observed an employee crack a raw whole shelled unpasteurized egg without gloves for a cocktail, wipe their hands with a napkin, and then proceed to scoop ice. Also observed an employee in the warewashing area change gloves without washing their hands between handling dirty and clean dishes. Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Person in charge indicated they would train staff when to wash their hands. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed raw fresh oysters, hiramasa crudo, and Ceasar salad made with a dressing containing raw unpasteurized egg on the menu without a disclosure symbol; these foods contain raw animal proteins. Offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Must provide approved consumer advisory at all points of sale prior to re-inspection. Corrective Action: Correct prior to re-inspection on 11/04/24.

Description: Contamination Prevented during Food Preparation, Storage and Display

Comments:

3-303.12 - Core: Storage or Display of Food in Contact with Water or Ice Addl Notes: Observed bags of retail cubed ice and a bottle of sparkling wine stored in ice in an ice well; in the adjacent ice well compartment, there is ice for consumption. The divider between the two wells does not completely separate allowing for commingling of storage ice and ice for consumption (observed gaps in the middle plate between the two well compartments. Person in charge indicated that the divider is meant to be easily removed and does not completely isolate both wells. Discussed installing a watertight divider if they want to use the well for ice for storage and ice for consumption. Also discussed only storing the retail cubed ice in the well with the storage ice due to it being in a watertight bag. Person in charge removed the bottles and retail ice bags and burned the ice. Corrective Action: Corrected at time of inspection.

Addl Notes: Person in charge was unable to provide chlorine or quaternary ammonia test strips at time of inspection. Observed quaternary ammonia is wiping buckets and two warewashing machines utilizing chlorine as a sanitizer. Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection 11/04/24.

No County legal action will result from this inspection.
*Observed reversed osmosis (RO) filtration units installed in the warewashing area and in the walk-in refrigerator. These is no observed approved testable continuous pressure backflow prevention assembly upstream of either unit. No copper piping observed. Person in charge indicated these units were installed 2 years ago. Discussed ensuring that there is an approved testable continuous pressure backflow prevention assembly between the RO units and any copper/copper alloys piping.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
