OC Inspection, E&D Class 4, 10+ Seating. Inspection conducted with site superintendent Ryan D. Establishment is not open at this time. OC permit is in conjunction with remodel permit RM-08898.

Correct the following items prior to final inspection:
1) Provide hot water to all sink fixtures throughout the establishment (100-110’F).
2) Ensure walk-in cooler, freezer and all other coolers are on and working properly. Cold holding temperature must be 41’F or below.
3) Provide a faucet that reaches all three basins for the 3-compartment sink in the bar area.
4) Finish connecting the under counter bar lights.
5) Provide a bead of silicone behind all sink fixtures.
6) Provide adequate lighting in the walk-in beer cooler.
7) Repair/replace the fan in the walk-in beer cooler.
8) Provide a dedicated water line at the mop sink for the chemical dispenser.
9) Provide a rigid drain line for the steam tables and route it to a floor sink with a 1-inch air gap. Ensure line is sloped to drain with a minimum 1/4-inch drop per horizontal foot.
10) Provide a funnel drain below the ASSE 1013 RPZ backflow device and route the drain line to a floor sink.
11) Test and certify the ASSE 1013 RPZ backflow device. Provide the test report.
12) Reseal in between the gaps of the hood with high temperature silver colored silicone.
13) Install the air curtain at the back door. Air curtain should be activated with a micro switch.
14) Replace/refurbish the shelving in the walk-in as discussed.
15) Provide clean out covers/escutcheon plates where missing.
16) Deep clean the walls, floors, ceiling, and equipment throughout.
17) Ensure the exhaust fan is working properly in both the men’s and women’s restroom.
18) Provide a trash can with a lid in the women’s restroom.
Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.
