Addl Notes: Observed raw liquid eggs received and accepted from commissary delivery at 47-48*F at the time of inspection. TCS food shall be received at 41°F or below; except Raw eggs can be received at 45°F or less; Cooked TCS food and received hot shall be at 135°F or above. Discussed with PIC the importance of taking temperature of TCS items before accepting them. PIC obtained log of temperatures taken from the commissary less than 4 hours prior to delivery and TCS foods were cooled to 41*F at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cambros of butter with milk as an ingredient on the cookline 79*F-81*F, individually portioned containers of butter in a reach in cooler 65*F, individually portioned TCS salsa with cut tomatoes 57-59*F and tzatziki sauce with yogurt as ingredient 58*F above the fill line of the cold table. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. PIC stated food items have been out of refrigeration since 7:30am, greater than 4 hours prior to inspection time. All TCS foods improperly cold held were discarded by PIC at time of inspection. Addl Notes: Observed RTE/TCS cooked sausage with 9/20 date mark at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). PIC was instructed to discard all RTE/TCS foods past 7 days of preparation at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed a container of Advil medicine stored above prep table on the cookline at the time of inspection. All medicines must be stored away from food, utensils, and single service items to prevent contamination. All medicines were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed in-use utensils by the waffle maker resting on a towel, tongs hanging on equipment handle on the cookline at the time of inspection. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Consecutive repeat: 3rd. Corrective Action: Corrected at time of inspection.

6-403.11 - Core: Designated Areas-Employee Accommodations for Eating/Drinking/Smoking Addl Notes: Observed 5 employee beverages and two open bowls of employee food stored on top of mechanical dishwasher at the time of inspection. Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens, and single-service and single-use articles cannot occur. Instructed PIC to have a designated area to prevent possible contamination. Consecutive repeat violation: 3rd. Corrective Action: Correct prior to next routine inspection.

Discussed with PIC: dishwasher at the bar has a work order in and is currently not in use, date marking, barrier exemption protocol for pests inside establishment, keeping handwashing sinks properly stocked.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
