Addl Notes: In walk in cooler, open flats of raw shell eggs stored over covered containers of ready to eat sauces and cooked meats. Instructed manager to move flats of raw shell eggs below ready to eat foods to prevent future cross contamination. All raw animal proteins are to be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. Corrective Action: Corrected at time of inspection.

Addl Notes: On cook line, pork belly/90*F and cooked onions/100*F-119*F out of internal temperatures. Per manager, pork belly and cooked onions were hot holding for less than 2 hours and ready for service. Pork belly and cooked onions were reheated to 165*F. Maintain TCS (time/ temperature control for food safety) foods at a hot holding temperature of 135*F or higher at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: In walk in cooler, chicken bones with meat/ 47*F and cooked meat sauce/ 46*F out of internal temperatures. Per employee, chicken and meat sauce were prepared two days ago and previous day. Chicken bones with meat and cooked meat sauce were discarded. See embargo form. On cook line, in metal containers sitting on ice, TCS foods (cut tomato mixtures, cut leafy greens, cooked vegetables) with internal temperatures of 46*F-55*F. Per manager, TCS foods were placed out for service <4 hours. Ice baths were made for TCS foods to rapidly cool back down to the proper temperature. Maintain TCS (time/ temperature control for food safety) foods at a cold holding temperature of 41*F or less at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: In hot holding box unit near back side of rotisserie, prime rib and shaved prime rib with internal temperatures of 90*F-110*F. Per manager, prime rib is cooked on rotisserie, which is unable to have FDA code parameters set. Per manager, prime rib is used for the evening and discarded. Establishment does not have written time control procedures and did not have start/discard time on hot holding box. Manager provided start/discard time for prime rib and time control procedures were written at time of inspection. Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Corrective Action: Corrected at time of inspection.

Addl Notes: Walk in cooler with ambient temperature of 46*F, taken with inspector probe thermometer. TCS foods with internal temperatures of 42*F-43*F. Repair company was called and on site by end of inspection. If ambient temperature of walk in cooler is unable to drop to appropriate temperature, TCS foods will be relocated to additional alcohol walk in cooler which has an ambient temperature of 37*F. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct prior to re-inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
