Informed PIC that a Certified Food Protection Manager needs to be available onsite during all hours of operation. A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed all hand sinks in the back of the establishment, including the employee bathroom to not have hand drying provisions available. Some machines were empty, and others had dead batteries. Informed PIC that all hand sinks need to be supplied with hand drying provisions at all times. PIC provided hand towels at time of inspection. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. PIC provided hand drying provision for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Facilities must be maintained in a condition that promotes handwashing. PIC provided soap for handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed two containers of raw pork longaniza (sausages) stored above tortillas and chopped raw vegetables. Informed PIC that the sausages cannot be stored above ready-to-eat food and produce. Discussed with PIC that the vegetables do not have a specific final cooking temperature, therefore they cannot be stored under the sausages. PIC properly stored the sausages and RTE foods at time of inspection. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed no consumer advisory on the menu post on the website or the mobile ordering site menu. They are missing the disclosure and reminder statement on the lunch/dinner menu and the brunch menu. The items on the menu that need the consumer advisory are the items with an undercooked egg available and any meat available to be served undercooked. For example, the chilaquiles de la casa and aguachiles. Provide full consumer advisory on the online menus prior to reinspection on 04/28/2025. Offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Must provide approved consumer advisory at all points of sale prior to re-inspection. Corrective Action: Correct prior to re-inspection on 4/28/2025. ***** 2nd Consecutive Violation *****

7-202.11 - Priority Foundation: Restriction-Presence and Use Addl Notes: Observed a can of Raid ant and roach spray in the employee bathroom. Informed PIC that pest control chemicals cannot be held onsite or applied by the establishment. If pest control is needed, it must be applied by a certified pest control specialist. The spray can was discarded at time of inspection. All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.

Description: Non-Food-Contact Surfaces Clean
Comments:
4-601.11(B) and (C) - Core: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils Addl Notes: Observed lava rock molcajetes in the kitchen used to serve steak, chicken, lobster, nopal, and salsas with dark discoloration on the inside and outer rims, as well as food debris stuck in some of the holes. PIC stated that they use foil as a barrier for the food. Informed PIC that the lava rock and all nonfood contact surfaces must be easily cleanable. Informed PIC of requirements for maintaining nonfood contact surfaces clean. Corrective Action: Correct prior to next routine inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
