PRIORITY VIOLATION-4-501.114 (A-E) - Priority: Manual and Mechanical Warewashing Equipment, Chemical Sanitization- Temperature, pH, Concentration and Hardness. Addl Notes: Observed chemical dishwasher in main bar with a chlorine concentration of 0 ppm chlorine at the time of inspection. Observed main bar dish machine in use at time of inspection. Person in charge (PIC) primed dishwasher and ran the machine additional times; no chlorine was observed from any of the runs. Establishment instructed not to use dish washer until it has a measure chlorine concentration of 50-100 ppm. Establishment agreed to use other bar dish machine/dish machine in the back of house until it is repaired.

Addl Notes: Observed salsa in the walk-in with a prepped date of 1/16 at time of inspection. Observed establishment made garlic and oil mixture with a prepped date of 1/2 & maple butter with a prepped date of 1/27 on top of prep table next to pizza oven at time of inspection. Establishment roasts fresh garlic in oven for garlic and oil mixture and melts butter on stove for maple butter. All items were embargoed at time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

Addl Notes: Observed skewered bratwurst with temperatures between 67 to 72*F in prep table across from the raw chicken prep table at time of inspection. Per cook, bratwust was prepped an hour prior to inspection. Cook moved bratwurst to walk-in cooler to rapid cool at time of inspection. Observed cooked chicken wings stored in deep containers, stacked with lids on and temperatures between 67 to 68*F at time of inspection. Per PIC items were cooked less than an hour prior. Cook placed chicken wings on sheet pans in the walk-in to rapid cool at time of inspection. Observed hard boiled eggs in reach-in cooler with lids on with temperatures between 56 to 57*F in a reach-in cooler next to the salad station at time of inspection. Per cook, eggs were cooked less than an hour prior to inspection & the cook removed lids from containers at time of inspection. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means.

Discussed with PIC about monitoring cold holding and equipment. Reviewed with PIC about not utilizing wet wiping clothes under cutting boards at time of inspection. Reviewed requirement to keep establishment made maple butter & garlic in oil mixtures cold at time of inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
