5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed section of condensate line inside the walk-in refrigerator that holds break and produce with a small hole in it. The condensate line was leaking small amounts of water at the time of inspection. The establishment shall maintain any plumbing throughout the establishment in good repair. Corrective Action: Correct prior to next routine inspection.

Comments:
6-501.12 - Core: Cleaning, Frequency and Restrictions Addl Notes: Observed accumulation of dirt, dust, and other organic matter along the walls and A/C condenser unit fans inside the walk-in refrigerator containing meat and dairy. The establishment shall clean the walk-in refrigerator at a frequency to prevent the accumulation of dirt and organic matter. Corrective Action: Correct prior to next routine inspection.
6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed multiple cracked floor tiles throughout the establishment. Also observed sections of tiled cove base missing inside the walk-in refrigerator holding meat and dairy. In addition, observed section of exposed, rough concrete around the floor sink near the 3-compartment sink. The establishment shall replace/repair any sections of missing/broken floor tiles in order to produce a smooth, easily cleanable surface. Corrective Action: Correct prior to next routine inspection.

Due to the violations noted on this inspection report, County legal action may result from this inspection.
Upon inspection, a remodel has taken place which may require an application and plans to be submitted, and/or additional permits. Please submit an application, supporting documentation, and fees within 10 business days or legal action may result pursuant to MCEHC Chapter 1, Regulation 4(e). Applications with supporting documentation can be submitted via the customer portal "Permit Center" online at maricopacountyaz-energovweb.tylerhost.net/apps/selfservice#/home.
- The establishment has installed a new combination walk-in refrigerator/walk-in freezer that is detached from the main building. Per owner, the walk-in was installed in December. Upon inspection, observed the light not working in the freezer portion of the walk-in. Also observed a build-up of ice along the door entering into the freezer. In addition, observed the outer opening of the walk-in refrigerator inadequately sealing shut, providing a gap/opening along the bottom corner of the door. -Per the owner, the establishment has plans to expand their grill area on the Eating & Drinking side of the establishment, requiring an expansion of the ventilation hood and associated fire suppression system. - At the time of inspection, observed the mop sink with a Do Not Use sign on it. Per manager, the establishment has been told by the City of Buckeye to discontinue dumping grey water containing degreaser & bleach down the mop sink drain. The manager stated the staff have been dumping used mop water outside per guidance of City of Buckeye. Requested establishment provide City of Buckeye contact information to gain clarification on the issue.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
