Addl Notes: 2nd Consecutive Violation. In the back prep cooler by the flat grill, observed cooked sausage, sliced cheese, and pooled eggs, all between 51*F and 55*F. Per manager, foods had been in the unit for approximately 2 hours. Foods were moved to the walk in to cool. In the front toppings cooler by the register, observed cabbage, pico de gallo, various tomato based salsas, sliced cheese, and various dairy based sauces at 45*F to 52*F. Foods had been in the unit for approximately 2 hours and were returned to the walk in cooler to cool. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Addl Notes: In the walk in cooler, observed 2 pans of cooked beans, tomato salsa, cheese sauce, and 2 pans of cooked rice all without date marks and prepared over 24 hours prior. All ready to eat (RTE), time/temperature controlled for safety (TCS) foods must have a date within 24 hours. Manager had foods date marked. Predefined Comment: All RTE/TCS foods were properly date marked at the time of inspection. Predefined Comment: RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

Addl Notes: In the walk in cooler observed cooked chicken, cooked pork, cooked beef, salsa verde, salsa rosa, creamy chipotle sauce, and birria broth all past 7 days. 3 bags of pork were from 5/27 and 1 was from 5/22. 2 bags of beef were from 5/27 and 1 was from 5/22. 2 bags of chicken were from 5/27. Verda and rosa salsas were from 5/26. creamy chipotle sauce was from 5/14. Birria broth was from 5/27. All foods were embargoed. Predefined Comment: RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Predefined Comment: All RTE/TCS foods past 7 days of preparation were discarded by PIC at the time of inspection. Corrective Action: Corrected at time of inspection.

7-202.11 - Priority Foundation: Restriction-Presence and Use Addl Notes: In the office, observed two cans of insecticide sprays. Pesticides cannot be stored at the establishment and all pesticides must be applied by a certified applicator. Manager removed pesticides from the establishment. Predefined Comment: All pest control for the establishment must be performed by a certified pest control specialist and chemicals for pest control must not be stored onsite. PIC removed pest control chemicals from premises at time of inspection. Corrective Action: Corrected at time of inspection.

4-602.13 - Core: Nonfood Contact Surfaces Addl Notes: 2nd Consecutive Violation. Observed food debris, residues, liquids, and trash accumulating inside the drawer prep unit under the flat grill. The inside of equipment must be cleaned frequently enough to prevent the accumulation of soils. Please have this area cleaned and cleaned more frequently. Predefined Comment: Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

PRIORITY VIOLATION-5-205.12 (A) - Priority: Prohibiting a Cross Connection; Drinking Lines

Addl Notes: 2nd repeat violation. Observed a sprayer nozzle attached to the hose on the mop sink downstream of the atmospheric vacuum breaker (AVB). Stoppage valves, such as sprayer nozzles, are not allowed to be kept downstream of AVB's when not in use as this poses a risk to breaking the AVB. Manager removed the sprayer nozzle at time of inspection. Predefined Comment: Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Corrective Action: Corrected at time of inspection.

"A" for Food Safety

Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.

How does it work?

The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.

How can I determine the grade?

The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.

Definitions

Priority Violation
Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
Priority Foundation Violation
Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
Core Violation
Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
Cutting Edge Program
All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
