A CPFM arrived approximately 20 minutes into the inspection. A CFPM must be present at all times of operations. Please have a CFPM present at all times of the next inspection. Addl Notes: Observed 3 covered pans of cooked rice at 105*F in the walk in cooler. Rice was cooked and started cooling 5 hours prior. Rice must be cooled below 70*F within the first 2 hours of cooling. Because it was still above 70*F after 2 hours of cooling, the rice was embargoed. Predefined Comment: Cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. Corrective Action: Corrected at time of inspection.

Addl Notes: 3RD REPEAT VIOLATION. Observed 2 pans of cooked carne asada at 110*F to 117*F on the prep line. Per manager the foods had been there approximately 1.5 to 2 hours. Carne asada was reheated on the flat grill above 165*F before returning to hot holding. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 135°F or above. Corrective Action: Corrected at time of inspection.

Addl Notes: In the drawer cold holding unit, observed soft cooked bacon, cooked chorizo, and cooked potatoes at 65-70*F. Per manager, the foods were prepared, cooled in the walk in cooler, and placed in the cold holding unit 3.5 hours prior. No temperatures were taken throughout the process. Foods were moved to walk in cooler to cool rapidly. Predefined Comment: Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed two large pans of cooked beans cooling in the walk in cooler. While cooling they had been covered the entire time. Beans were at 51*F and were cooked and placed in cooler 4 hours prior. Discussed proper cooling methods with manager. Beans were uncovered. Predefined Comment: All TCS foods cooling were uncovered to allow for rapid cooling. Corrective Action: Corrected at time of inspection.

6-501.111 (C) - Priority Foundation: Controlling Pests; Approved Trapping Methods/Existence Addl Notes: Observed 30+ drain flies in the back by the mop sink and dirty towel basket. Establishment must remain free of pests at all times. Please have pest control come treat for the drain flies. Inspector will follow up with a re-inspection within 10 days. Predefined Comment: Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection.

PRIORITY VIOLATION-5-205.12 (A) - Priority: Prohibiting a Cross Connection; Drinking Lines
Addl Notes: Observed a Sprayer nozzle with the ability to stop the flow of water on the mop sink hose. Sprayer nozzles with the ability to stop the flow of water must be removed from the hose when not in use. Nozzle was removed at time of inspection. Predefined Comment: Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Corrective Action: Corrected at time of inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
