Addl Notes: At in use 3-compartment sink, employee washed and rinsed dishware without a proper final sanitizer rinse. Employee was instructed on proper 3-compartment sink set up and final rinse procedures. Chlorine sanitizer rinse was made with a concentration of 50-100ppm. Corrective Action: Corrected at time of inspection.

Addl Notes: On tables, large mouth containers with lids and spoons, filled with chili sauce, left on tables and re-used from customer to customer. Employees removed large mouth containers of chili sauce. Discussed using single use containers or narrow necked bottles that can remain on tables for re-use. (B) Except as specified under ¶ 3-801.11(G), a container of FOOD that is not TIME/TEMPERATURE CONTROL FOR SAFETY FOOD may be RESERVED from one CONSUMER to another if: (1) The FOOD is dispensed so that it is protected from contamination and the container is closed between uses, such as a narrow-neck bottle containing catsup, steak sauce, or wine; or (2) The FOOD, such as crackers, salt, or pepper, is in an unopened original PACKAGE and is maintained in sound condition. Addl Notes: On cook line, cooked bok choy, cooked fried rice, and sprouts with internal temperatures of 50*F-85*F. Per employee, TCS foods were prepared <30 minutes. Cooked bok choy, cooked fried rice, and sprouts were moved to prep table cooler to rapidly cool back down to the proper temperature. Discussed using ice to maintain cooked foods at proper temperature when removed from prep table cooler. Maintain TCS (time/ temperature control for food safety) foods at a cold holding temperature of 41*F or less at all times. Corrective Action: Corrected at time of inspection.

4-602.13 - Core: Nonfood Contact Surfaces Addl Notes: Throughout kitchen, on doors of microwave, doors and handles of prep table coolers, swinging door from cook line to back of kitchen with accumulation of food residues. Maintain clean. Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

No County legal action will result from this inspection.
Joint inspection conducted with USDA SITC officer, Chinese liaison, and district inspector.
Recommendations from inspectors: re-print menu to include both English and Chinese on all food items and remove duck blood option.
Currently, establishment is under old ownership and will be changing ownership at end of the summer. Current manager/ future new owner owns another restaurant and cooks duck at that location and sells at this location. Due to different ownership at this time, the cooked duck from other establishment is not allowed to be sold at this location.
Via email, provided information on: Chinese handouts for handwashing, warewashing, storage, bodily fluid procedures, symptoms and diseases associated with food borne illnesses.
Also provided new ownership application for future ownership change.
Ensure all employees working with food obtain food worker certification from ANSI certified online site.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
