Comments/Details
1). Ply wood and base has been sealed in the back room.
2). Any stucco wall behind racking for storage needs to be painted out and block filled if needed when not cleanable.
3). Walls with tracking for food storage, areas with equipment like RO system etc. needs to be based out.
4). Area under the trash Shute needs to be finished out with coved base at the floor / wall juncture.
5). In the bathrooms or anywhere occurring like at tile work, where FRP or tile is cut to wide around escutcheon plates and outlets, junction boxes, fill in with caulk and finish out.
6). All floor sinks throughout the store will need to be accessible for cleaning. Next to that, the floor sinks need to be positioned near the edge or half out from under the toe kicks. When not achievable the toe kick will need to be removable or outfitted with a cut-in and with piano hinge.
7). In the back storage room, where the drain lines etc, are entering the floor, make sure to fill in the gaps around the pipes.
8). All refrigeration cases positions against the walls on the sales floor will need to be flashed to the back walls. You can skip the one in the meat market as only staff goes in and out of that area.
Please provide the following backflow valves:
9). Install a spill proof pressure vacuum breaker upstream of the spray down proportioner. Install 12 inches above the dispenser.
10). For condensate line of the produce cooler, see if you can run the condensate line to the floor sink following contours of the wall. Don't cross over line now.
Install RP valves with hub drain assembly and note that testable valve(s) will need to be tested and certified before final inspection.
11). Unisex restroom in the SE corner is missing a fire escutcheon.
12). Noted several unprotected drain lines in the back storage area sticking out beyond the base or exposed along the wall. Not sure if you will install shielding but it is recommended in areas where shelving is not installed.
13). Please base out the wall and see if you can silicone the areas where the ice bagger is going to be installed and where the produce RO will be installed.
14). In the dairy cooler also install base along the access door wall is. Base is in place along all other walls.
15). Multiple rooms will be based out, mop room NW corner, SW corner, drive up and go room, retail floor where visible. Metal base will be used for walk-in walls.
16). Produce cooler and prep cooler will receive stainless coved base.
17). In the produce prep room, please caulk, vertically the seam between the ISP panel and the FRP.
18). Silicone stainless corners where open.
19). Provide an air gap in the eductor of the veggie wash dispenser.
20). Doors will be installed between the produce prep area and the hall way.
21). Store will be based out along laminate wall panels.
All testable backflow valves will require testing and certification before final inspection.
Store is okay to stock the coolers and some of the dry goods after city approval.
Coolers are holding temperature and have been on.
No County legal action will result from this inspection.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
