Addl Notes: Observed raw chicken being stored over tamatillos in the walk in cooler. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. Employee properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection. 2ND REPEAT VIOLATION.

3-501.17 - Priority Foundation: Ready-To-Eat Time/Temperature Control for Safety Food, Date Marking. Addl Notes: Throughout the establishment, observed chiles rellenos, in house salsa, cooked beans, and in house pickled carrots/peppers with no date marking. Employee states all these foods were opened/prepped/cooked 1-2 days prior to the inspection except for the pickled carrots/peppers which was unknown when it was made. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods were properly date marked at the time of inspection except for the pickled carrots which were voluntarily disposed of. Corrective Action: Corrected at time of inspection. 2ND REPEAT VIOALTION.

Addl Notes: Observed cooked beans cooling in the walk in cooler but being cooled in deep buckets (more than 4 inches). Person in charge states they had been cooling for less than 1 hour. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. All TCS foods being cooled in deep containers were transferred to shallow containers to allow for rapid cooling. Corrective Action: Corrected at time of inspection.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed kitchen tongs used to handle raw animal products having its handle in contact with the food. Also observed a scoop having its handle in contact with the pico de gallo. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

4-502.13(A) - Core: Single-Service and Single-Use Articles-Use Limitations Addl Notes: Observed the establishment re using plastic containers that the sour cream was originally packaged in to store other TCS/Non-TCS foods such as in house hot sauce and salsa, and cooked beans. Discussed the importance of not re-using any single-use containers to prevent any contamination due their inability to safely withstand numerous washing/sanitizing cycles. Corrective Action: Correct prior to next routine inspection. 2ND REPEAT VIOALTION.

6-501.114 - Core: Maintaining Premises, Unnecessary Items and Litter Addl Notes: Observed several food equipment in the dry storage area such as meat cutters that were not NSF approved equipment which ensures they are made of food grade material and are easily cleanable. Please remove any equipment or utensils that are not NSF approved from the facility. Corrective Action: Correct prior to next routine inspection. 2ND REPEAT VIOLATION.

"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
