Addl Notes: Observed raw whole eggs and egg whites in cartons located in walk in fridge stored above boxes of bread. Raw fish stored above cooked chicken in walk in fridge as well as raw chicken above intact raw beef. Observed in stand up fridge near cook line entrance raw turkey bacon and raw beef stored above cooked brisket, lettuce, chili sauce, and egg whites. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed cooked brisket in walk in shallow metal dish with the top covered in tinfoil with a slit in foil. Brisket temperature ranged from 43°F- 47°F depending on thickness of meat and proximity to opening in foil. Upon questioning, person in charge stated brisket had been cooked around 8pm the previous day and put in walk in fridge afterwards. Cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. Explained to staff to remove tinfoil and place on top shelf in fridge to help food facilitate cooling in the future. All TCS foods improperly cooled were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed facility to have no date marking system in place. Establishment had cut tomatoes, cut lettuce, cooked animal products, opened cheeses, RTE cheesecakes, among other RTE/TCS foods with no date marks. Explained to staff importance of date marking and what food items require it. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods were properly date marked at the time of inspection. Corrective Action: Corrected at time of inspection.

4-302.12 - Priority Foundation: Food Temperature Measuring Devices Addl Notes: Observed establishment to have infrared thermometer. Person in charge was not able to locate a probe thermometer at time of inspection. Properly calibrated probe thermometers are required at all times for monitoring food temperatures. Must provide a probe thermometer for establishment prior to re-inspection. Corrective Action: Correct prior to re-inspection. Re-inspection scheduled for 4/15/2025.

3-304.12 - Core: In-Use Utensils, Between-Use Storage Addl Notes: Observed in-use spatulas stored in water at 75*F. In-use utensils must be stored in a clean, dry location; in running water; or in hot water above 135°F. If utensils are stored in food, the handles must be stored above the surface of the food. Cook moved metal container with water onto stovetop to keep water at 135*F for utensil storage. All in-use utensils were properly stored at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed three comp sink to have auto dispenser for quat sanitizer as well as chlorine bleach to be present at establishment. Person in charge could not locate test strips for quaternary ammonia nor chlorine. Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection. Re-inspection scheduled for 4/15/2025

4-601.11(B) and (C) - Core: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils Addl Notes: Observed inside of microwave to have food debris build up, inside of prep fridges along cook line had food debris build up. As well as fridge door handles observed to have food and dirt accumulation. Nonfood-contact surfaces shall be kept free of an accumulation of dust, dirt, food residue, and other debris to prevent pathogenic growth. Nonfood-contact surfaces shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

Auto dispenser of quat sanitizer for three comp sink did not pull sanitizer at proper concentration. Instructed staff to self dilute chlorine bleach to sanitize food contact surfaces until dispenser is fixed.
Discussed with staff that butter is a TCS item and shall be kept at 41*F or lower, recommended using time as a control and provided handout for 4hr time as a control. Reviewed food separation, proper cooling of cooked foods, proper sanitization concentration and frequency of sanitization of food contact surfaces. Provided date marking and TCS foods handout via email.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
