Addl Notes: Observed raw chicken, lamb and goat stored in thin plastic bags above RTE herbs, rice, and cooked chicken curry. Observed raw chicken stored above raw marinating lamb. In the home style fridge labeled "personal food only" observed raw eggs stored above RTE yogurt and raw ginger that was intended for service to customers. All raw animal proteins must be stored below ready-to-eat (RTE) foods and stored according to cooking temperature at all times to prevent cross contamination. PIC properly stored all raw animal proteins according to the final cooking temperature based on the cooking parameters in § 3-401.11 of the Food Code. Corrective Action: Corrected at time of inspection.

PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed RTE TCS foods without date marks including: cooked chicken curry, cooked rice, various sauces, and housemade pickles. PIC contacted the employee that prepared the foods who stated they were prepped on 10/10. In the freezer, observed various taco ingredients that were cooked and cooled without dates marks including carne asada, chicken, beans and frozen burritos. Also observed frozen tandori chicken without date marks from the day of prep. All RTE/TCS foods were properly date marked at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Corrective Action: Corrected at time of inspection.

Addl Notes: Observed RTE potato wedge fries at 96*F, bone in chicken and chicken tenders between 107-112*F, and mac and cheese at 122*F. PIC stated the foods were on time as a control but was too busy to add timestamps. PIC stated the foods were cooked between 12:30-1pm. All TCS foods being held under time as a control were properly time stamped at time of inspection (12:30pm). Time as a Public Health Control requires written procedures detailing process for Time as a Public Health Control and time-stamping for items actively held under Time as a Public Health Control. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed Pepto stomach medicine for personal use stored in the personal fridge above whole ginger and yogurt intended for service to customers. All medicines must be stored away from food, utensils, and single service items to prevent contamination. All medicines were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed chemical burners stored above RTE spices on the rack next to the personal cooler. Observed oven cleaner stored next to portion cups. Observed bleach and window cleaner stored above foodservice gloves. Chemicals must be stored away from food, utensils, and single service items at all times to prevent contamination. All chemicals were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: No sanitizer test strips observed for quat or chlorine. Establishment has both sanitizers available but no test strips available to verify sanitizer concentration. Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection.

No County legal action will result from this inspection.
Establishment was observed operating outside the scope of their approved operating permit in violation of MCEHC Chap. 1, Reg. 4(a). Establishment is operating as a Class 4 E&D 0-9 Seating food establishment. Observed multiple foods prepared with advanced cooling processes including various sauces, rice, cooked chicken, as well as various cooked meats from tacos including beef, chicken and beans. Advised PIC a new fee may be assessed on the next billing cycle as well.
Observed excessive amount of flour lumps in the chicken batter stored at room temp due to lumps not being sifted out and discarded. PIC stated they wanted to dry out the lumps and blend into a powder again to be re-used. Advised PIC that battering station clumps must be regularly sifted out and discarded. Clumps may not be dehydrated and repurposed.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
