A Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed food debris on the inside of a metal pan and 2 food dicers stored as clean. All food-contact surfaces must be cleaned to sight and touch. PIC relocated all food-contact surfaces to warewashing area to be washed, rinsed, and sanitized at the time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed mozzarella at 47*F, American cheese slices at 44*F, and shredded cheese at 45*F when measured with a probe thermometer next to the grill. Observed the foods stored above an ice bath that had melted with water at 41*F. Per PIC, the cheeses had been replenished during lunch and had been in the ice bath for approximately 3 hours. All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Reviewed ice bath setup with the PIC. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed a bottle of ibuprofen stored in a plastic container above the prep cooler on the cook line. All medicines must be stored away from food, utensils, and single service items to prevent contamination. All medicines were relocated and properly stored by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

6-202.15 - Core: Outer Openings, Protected Addl Notes: Observed the back door to the establishment ajar at the time of inspection. Observed the door in disrepair so that the door does not self-close easily. Advised PIC to repair door to easily close and tightly seal when not in use. Advised PIC propping door open may allow for pest entry. Corrective Action: Corrected at time of inspection.

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed multiple drain lines stored below the flood rim of the floor sink servicing the drain lines. Indirectly plumbed drain lines must be stored 1 inch above the flood rim. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

5-501.115 - Core: Maintaining Refuse Areas and Enclosures Addl Notes: Observed debris and oil spilled around the trash and grease receptacles in the outdoor trash area. PIC stated the same trash is use by two other food establishments. Advised PIC to have the area cleaned to ensure the trash area does not attract pests or become a nuisance. Corrective Action: Correct prior to next routine inspection.

No County legal action will result from this inspection.
Observed a corn tortilla roller in use with exposed gears and areas that appeared difficult to clean. Reviewed cleaning procedure with PIC who stated the machine is only cleaned using water. Advised PIC equipment used with foods must be NSF or equivalent and must be washed, rinsed and sanitized every 24 hours when used with non-TCS corn masa. No housing or cover was observed to protect the gears and moving elements which were observed with a buildup of food debris. PIC did not have information from the manufacturer or proof of the machine being NSF. PIC will contact their supplier and provide a spec sheet and any available information. If the machine is not NSF or equivalent, discussed discontinuing use of this machine and purchasing one that is approved.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
