Addl Notes: Observed dishwasher handling and washing dirty dishes, then cleaned the floor, and then proceed to handle cleaned and sanitized dishes without washing hands. Discussed proper hand washing with PIC. Hands must be washed after handling dirty dishes before handling cleaned and sanitized dishes regardless if the dish washer is wearing gloves or not. Dishwasher washed hands for 20 seconds at the time of inspection before continuing to put away clean dishes. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed hand wash sink near the dish machine without hand drying provisions. Employee restocked hand wash sink with paper towels at time of inspection. Each handwashing sink must be provided with hand drying provisions at all times. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed the hand wash sink closest to the entrance to the kitchen with a sanitizer bucket sitting in the basin of the hand wash sink. Also observed a pot sitting in the basin of the hand wash sink located near the dish machine. Employee removed the items blocking both hand wash sinks at time of inspection. A handwashing sink must be maintained so that it is always accessible for employee use and may not be used for purposes other than handwashing. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed green chilies in a container located in a reach in cooler visibly adulterated with mold. Discussed methods to prevent adulteration with PIC. All food must be in good condition, safe, and unadulterated. All adulterated food was discarded at time of inspection. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed molcajete bowl being used to store and make salsa for the breakfast line. The molcajete bowl was removed from the line at time of inspection, and PIC will discontinue use of the bowel. All food-contact surfaces must be smooth, non-absorbent, and easily cleanable. Corrective Action: Corrected at time of inspection.

Addl Notes: Observed establishment without testing strips for quaternary ammonium used at the 3 compartment sink. Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection. Corrective Action: Correct prior to re-inspection on 7/6/2026.

Discussed maintaining individually wrapped butter and cream cheese cold if the establishment is planning on reusing product, recommended providing a high temperature thermometer for accurately measuring the rinse cycle of the high temp dish machine, rising the drain lines for the dish machine, provided a guidance document for creating a time control policy, and provided a 1022 dual check valve along the water line for the juice machine.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
