Maricopa County requires all establishments that serve open food, which requires time/temperature control for food safety, to have at least one certified Food Protection Manager (CFPM) on staff. The Person in Charge shall be a CFPM and is required to be onsite during the hours of operation. This certificate must be visible or readily accessible. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed employee pick items up off of the floor at the cook-line, change gloves, then handle clean utensils to place food into single-use containers for immediate service to customers. Observed employee sweeping on the cook-line and handle other cleaning tools on the cook-line and prep area without washing hands prior to handling food and clean utensils. Discussed the importance of handwashing and when handwashing is required. According to PIC, the employee is still in training and will receive additional handwashing training. Any activity which may contaminate the hands must be followed by thorough handwashing. Corrective Action: Corrected at time of inspection.

4-602.11 (E) - Core: Equipment Food - Contact Surfaces and Utensils-Frequency; Non-TCS. Addl Notes: 2ND CONSECUTIVE VIOLATION Observed the onion dicer/punch stored as clean under the prep table with an accumulation of dried onion and soil on the blades. PIC will ensure that the onion dicer is cleaned prior to use and will inspect the blades and any food contact surfaces prior to storing as clean. Also observed an accumulation of soil/organic substance on the inside food contact surfaces of the ice machine directly behind the cook-line wall. Discussed cleaning the inside of the ice machine more often. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Correct prior to next routine inspection.

Addl Notes: Observed employee wash, rinse, and sanitize soiled utensils. The utensils were sprayed with the quat sanitizer hose and immediately placed back onto the cook-line to be used. Instructed employee to fill the sanitize basin of the 3-compartment with enough sanitizer to submerge the utensils for 60 seconds (according to manufacturer's instructions). PIC will retrain employees to submerge all equipment in the sanitizer, for the time specified, after washing and rinsing. Food-contact surfaces sanitized using quaternary ammonia must be sanitized with a contact time according to manufacturer's specifications to effectively destroy pathogens. Corrective Action: Corrected at time of inspection.

4-601.11(B) and (C) - Core: Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils Addl Notes: Observed a heavy accumulation of soil and debris in the utensil drawer of the prep table near the 3-compartment sink. PIC is aware that the drawer is soiled and will perform a deep clean. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Corrective Action: Correct prior to next routine inspection.

Discussed the mop sink drain and what appears to be clean-out drains in the concrete outside of the rear exit door. According to PIC, the floor drain next to the mop sink was recently serviced. Please provide covers for the clean-outs outside of the rear door and continue to monitor the mop sink and adjacent drain. The establishment should not operate if sewage begins to spill onto the floor from the drains.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
