4-402.11 - Core: Fixed Equipment, Spacing or Sealing-Installation Addl Notes: Corrective Action: Correct prior to next routine inspection. Noted ice condensation is forming on the condenser pipes and the fans' housing of the walk-in freezer. Please repair/ rewrap the condenser pipes and check for gaps, on walls, ceiling or door of the walk-in freezer. Please seal gaps with silver color low temp silicone. All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection.

5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Corrective Action: Correct prior to re-inspection. Please ensure all water connected, non-carbonated beverage dispensers must have an approved backflow device such as an inline dual check meeting ASSE 1022 standards. These are not testable devices, so replace them annually or whenever they leak; whichever comes first. Another option is to provide a RPZ (reduced pressure zone) assembly at the water connection point of the non-carbonated beverage dispensers and have the RPZ tested annually. Provide test reports for all testable backflow preventers inside the facility prior to re-inspection. All testable backflow devices must be certified annually or after a repair. Maintain records on file for inspection purposes. Noted the faucet at the 3-comp sink is loosen. Please anchor the faucet to the wall. All plumbing must be maintained in good repair at all times. Noted, the drainpipes for the shake machine and frappe machine are made of hoses and the hoses are laying inside the floor sink without proper air gap. Please replace the hoses with rigid drainpipes (PVC or ABS) and indirectly discharge to the nearest floor sink with minimum 1" air gap from the finished floor.

You are hereby notified that you are operating this establishment in violation of the MCEHC. Per Chapter 1, Regulation 4(a) "No person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so, unless otherwise specified in this Environmental Health Code." County legal action may result due to violations observed during this inspection. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.
This establishment/operation has demonstrated sufficient compliance to proceed with the permit issuance. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so.
"A" for Food Safety
Through our grading system, Maricopa County Environmental Services Department provides at-a-glance indications of how food establishments perform during food safety inspections. This system is designed to grade food establishments in terms of food safety requirements and provide recognition to those who choose to participate.
How does it work?
The issuance of grade cards to food establishments is based on voluntary participation. The person in charge, who is present during the inspection, will be asked whether they would like to participate in the Grading System. This will be decided prior to each inspection. If an establishment does not participate in the Grading System, Not Participating or NP will appear on the inspection report, and the inspection will still occur. If an establishment does participate, a grade of A, B, C, or D will be determined at the conclusion of the inspection using the Grading System chart below. Participating establishments will then be provided with a grade card that can be posted in the establishment. The status of the establishment’s grade will also appear on the Restaurant Ratings page of the department’s website.
How can I determine the grade?
The Grading System chart can be used to determine grades earned for establishments that do not participate. Grades are determined by the number of Priority and Priority Foundation violations received at the time of inspection. Violation types are defined in general terms below.
Definitions
- Priority Violation
- Major violation that directly contributes to increasing the risk of foodborne illness or injury (e.g. Food employees do not properly wash hands when required).
- Priority Foundation Violation
- Minor violation that does not directly contribute to an increased risk of foodborne illness but failure to correct this violation may lead to the occurrence of a priority violation. For example, hand washing soap and paper towels not available at hand wash sink (may lead directly to food employees not properly washing hands when required).
- Core Violation
- Minor violation that relates to general maintenance and sanitation (e.g. No sign reminding employees to wash hands).
- Cutting Edge Program
- All county food-related establishments can partner with Maricopa County to implement a proactive approach to reducing foodborne illness risk factors by implementing food safety systems that include policies, training, and verification processes. Partners enrolled in the Cutting Edge Program receive an A+ when an A is earned.
